Here's the key thing to know: in this pairing, it's the THC that gets affected, not your Apalutamide (Erleada). Apalutamide is a known booster, or 'inducer,' of the liver enzyme CYP2C9, and that same enzyme helps break down THC. So when the two are on board together, your body may clear the THC faster than usual, which can leave you with a weaker or shorter effect than you'd expect.
In plain terms, your cannabis might just not work as well while you're on Apalutamide. The good news is that your prostate cancer treatment isn't being weakened here. CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Here's the situation with Erleada (apalutamide) and THC. Apalutamide is partly broken down by a liver enzyme called CYP3A4, and some lab studies hint that THC might mildly slow that enzyme down. In theory, slowing it could let a bit more apalutamide build up, which might mean more of its usual side effects like fatigue, skin rash, joint pain, or high blood pressure.
That said, I want to keep this in perspective. This is rated a minor, unlikely interaction based only on test-tube data, and a human study actually found that oral THC did not block CYP3A4 at all. So real-world impact is probably small. Apalutamide is a serious cancer medicine, though, so before adding any THC product, please loop in your oncologist or pharmacist first.

What Is Apalutamide Used For?
Brand name: Erleada
Erleada (apalutamide) is used to treat two types of prostate cancer. The first is metastatic castration-sensitive prostate cancer (mCSPC) — prostate cancer that has spread to other parts of the body and still responds to hormone-lowering therapy. The second is non-metastatic castration-resistant prostate cancer (nmCRPC) — prostate cancer that has stopped responding to standard hormone therapy but has not yet spread to other organs.
Erleada is always used alongside a gonadotropin-releasing hormone (GnRH) analog — a medicine that lowers testosterone — or in patients who have already had surgery to remove the testicles.
