Acetaminophen, Chlorpheniramine, Pseudoephedrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Echinacea interacts with.
Echinacea appears to inhibit CYP1A2 enzymes in humans. Additionally, echinacea seems to increase plasma concentrations of caffeine, a CYP1A2 substrate, by around 30%. Theoretically, echinacea might increase levels of other drugs metabolized by CYP1A2.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This is one of those combinations that looks scarier on paper than it usually plays out in real life. Echinacea can gently slow down a liver enzyme called CYP1A2, and in studies it nudged up blood levels of caffeine by about 30 percent. The thinking is that it could do the same to other drugs that lean on that same enzyme.
Here's the practical part: of the three ingredients in your cold and allergy product (acetaminophen, chlorpheniramine, and pseudoephedrine), none rely heavily on CYP1A2, so any real-world effect is likely small. None of these are prodrugs, so they won't lose their punch. If levels did creep up at all, you might notice a bit more drowsiness, dry mouth, or jitteriness. For short-term use, this is usually low concern, but check with your pharmacist if you take it often.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Acetaminophen, Chlorpheniramine, Pseudoephedrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Several clinical trials have shown that taking echinacea for up to one month does not significantly affect the metabolism of various CYP3A4 substrates, including midazolam, docetaxel, etravirine, lopinavir-ritonavir, and darunavir-ritonavir. However, other clinical research shows that echinacea may increase the clearance of midazolam, suggesting that echinacea might induce CYP3A4. The discrepancy is thought to be due to differing effects of echinacea on intestinal versus hepatic CYP3A4 enzymes. Echinacea appears to induce hepatic CYP3A4 but inhibit intestinal CYP3A4. In some cases, these effects might cancel each other out, but in others, drug levels may be increased or decreased depending on the level of effect at hepatic and intestinal sites. The effect of echinacea on CYP3A4 activity may differ depending on the CYP3A4 substrate.

This is a combination cold and allergy product, and only part of it leans on the CYP3A4 enzyme that echinacea may nudge. The honest answer here is that the effect is mixed and pretty mild. Echinacea seems to speed up liver CYP3A4 while slowing the gut version, so for most people these effects roughly cancel out. None of these three ingredients is a prodrug, so you are not likely to lose or gain much benefit.
In the rare case levels shifted, you might notice a touch more drowsiness or dry mouth from the chlorpheniramine, or more jitteriness from pseudoephedrine. The evidence is modest and short-term use is usually fine. Still, run it by your pharmacist, especially if you have high blood pressure or take other medicines.
