Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Feverfew interacts with.

Theoretically, feverfew might increase levels of drugs metabolized by CYP2C9.
Laboratory research shows that feverfew might inhibit CYP2C9. So far, this interaction has not been reported in humans.

Opsynvi combines macitentan and tadalafil, taken together to treat pulmonary arterial hypertension (high blood pressure in the lungs). Both of these are active drugs, not prodrugs, and tadalafil in particular is partly cleared by a liver enzyme called CYP2C9. In a test tube, feverfew appeared to slow that enzyme down. If that happened in the body, it could theoretically let levels build up a bit, which might make side effects like headache, flushing, low blood pressure, dizziness, or a stuffy nose more noticeable.

Here's the honest part: this is only lab research, and it has never been reported in real people. So I wouldn't lose sleep over it, but since this is a serious condition, please run feverfew past your doctor or pharmacist before starting it.

Opsynvi combines two medicines, macitentan and tadalafil, and both are cleared by a liver enzyme called CYP3A4. Feverfew showed in lab dishes that it might slow this enzyme down. If that happened in the body, more of the drug could linger, which in theory could nudge up its effects and side effects. For tadalafil that can mean headache, flushing, stuffy nose, dizziness, or a bigger drop in blood pressure. Macitentan can add to lightheadedness and swelling.

Here is the reassuring part: this is only test-tube data and has never actually been seen in a real person. So the concern is theoretical, not proven. Still, since Opsynvi affects your blood pressure, please check with your pharmacist or doctor before adding feverfew rather than starting it on your own.

Feverfew is a daisy-family herb best known for migraine prevention, where some studies suggest it may modestly lower how often migraines occur, though the evidence is mixed.
