Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES
Why this applies to Tretinoin, Benzoyl Peroxide
Tretinoin, Benzoyl Peroxide falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES — the group Feverfew interacts with.
Laboratory research shows that feverfew might inhibit CYP2C8. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the good news on Twyneo (topical tretinoin with benzoyl peroxide) and feverfew: this is a cream you smooth onto your skin, not a pill you swallow. Even though tretinoin can be processed by the CYP2C8 enzyme that feverfew might block in a lab dish, very little of a topical product gets into your bloodstream. So this theoretical concern really doesn't translate into much for skin treatment.
On top of that, this interaction has only been seen in laboratory studies, never reported in actual people. I wouldn't lose sleep over it. The redness, peeling, or dryness you might notice from Twyneo comes from the cream itself, not from feverfew. If you'd like extra peace of mind, just run it by your pharmacist or doctor.

From our pharmacist’s overview on the Tretinoin, Benzoyl Peroxide page
“Twyneo pairs tretinoin, a vitamin A derivative, with benzoyl peroxide in an acne cream, and its interaction list deserves genuine care. The single major-rated entry among the 35 supplements on file is vitamin A itself; stacking supplements on a drug built from vitamin A can push levels toward toxicity, so that pairing is the one to und…”

About Feverfew
What is Feverfew?
- Migraine prevention
- Headache
- Fever (traditional)
- Arthritis and joint pain
- Menstrual discomfort
Feverfew is a daisy-family herb best known for migraine prevention, where some studies suggest it may modestly lower how often migraines occur, though the evidence is mixed. It is generally well tolerated for short-term use in most adults but should be avoided during pregnancy and breastfeeding and by people allergic to ragweed and related plants. Talk to your pharmacist or doctor before starting it.
