Galacto-oligosaccharides (gos) and Elranatamab

Here is the interaction we found between these two, with the evidence behind it.

IMMUNOSUPPRESSANTS

Why this applies to Elranatamab

Elranatamab falls under IMMUNOSUPPRESSANTS — the group Galacto-oligosaccharides (gos) interacts with.

Theoretically, GOS might reduce the effects of immunosuppressants.
Human and animal studies suggest that GOS may stimulate the immune system. This could theoretically alter the effects of immunosuppressant medications.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Elranatamab (Elrexfio) is a powerful antibody treatment for multiple myeloma. It works by pulling your immune system's T-cells toward cancer cells so they can attack them, so in a sense it's meant to activate part of your immune system in a very targeted way. The concern with GOS here is different and, honestly, pretty theoretical.

The idea is that GOS, by feeding gut bacteria, might gently nudge your overall immune system. On paper, that could work against medicines that quiet the immune system. But this is based on lab and animal signals only, not real cases with Elranatamab, so I wouldn't lose sleep over it. Since Elranatamab is a serious cancer therapy, please run any new supplement past your oncology team before starting.

From our pharmacist’s overview on the Elranatamab page

“Elranatamab (Elrexfio) fights multiple myeloma by recruiting the immune system against the cancer, and that mechanism is exactly why I want people on it to read this page carefully rather than skim it. Most of the interactions here involve herbs and mushroom extracts that stimulate immune activity, which could in theory interfere with…”
