Tolvaptan falls under P-GLYCOPROTEIN SUBSTRATES — the group Hesperidin interacts with.

Theoretically, hesperidin might inhibit P-glycoprotein-mediated drug efflux and potentially increase levels of drugs that are substrates of P-glycoprotein.
In vitro research shows that hesperidin can inhibit P-glycoprotein efflux. This effect has not been reported in humans.

Here's the situation with hesperidin and your tolvaptan. Tolvaptan is moved around and pushed out of your cells partly by a tiny pump called P-glycoprotein. In lab studies, hesperidin (a citrus flavonoid) seems to slow that pump down. If that same thing happened in your body, more tolvaptan could hang around, which in theory could make its effects stronger, things like increased urination, thirst, dry mouth, and dizziness.

The good news: this is only a theory based on test-tube research, and it has not actually been seen in people. The amount of citrus you eat normally is very unlikely to matter. Still, since tolvaptan needs careful dosing, it's worth a quick chat with your doctor or pharmacist before adding a concentrated hesperidin supplement.

Tolvaptan (brand name Jynarque) is used to slow the decline of kidney function in adults who have autosomal dominant polycystic kidney disease (ADPKD) — a genetic condition where fluid-filled cysts grow in the kidneys over time, gradually damaging them. It is specifically for people whose disease is at risk of progressing quickly.

It does not cure ADPKD, but it works to slow down how fast the kidneys deteriorate, helping preserve kidney function for longer.
