Why this applies to Febuxostat
Febuxostat falls under HEPATOTOXIC DRUGS — the group Niacin interacts with.
Niacin has been associated with cases of liver toxicity, especially when used in pharmacologic doses. Sustained-release niacin preparations appear to be associated with a higher risk of hepatotoxicity than immediate-release niacin.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: both febuxostat and high-dose niacin can, on their own, put some stress on the liver. Febuxostat carries a known (though uncommon) risk of liver enzyme changes, and pharmacologic doses of niacin have been linked to liver irritation too. When you stack two things that lean on the liver, the worry is that the combined load could raise the chance of liver trouble.
I want to be honest that this is a theoretical concern based on how these substances behave, not something proven to happen often. The everyday vitamin amount of niacin in food or a multivitamin isn't the issue; the concern is concentrated, drug-strength niacin (especially sustained-release). Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before combining them.

Niacin (vitamin B3) is an essential nutrient your body needs for energy and metabolism, and deficiency is uncommon in most developed countries. Prescription-strength niacin has been used to adjust cholesterol levels, but high doses can cause flushing and other side effects and should only be taken under medical supervision. About Febuxostat
What Is Febuxostat Used For?
Brand name: Uloric
Febuxostat (available as tablets and a dissolving oral film) is used to chronically manage hyperuricemia — high uric acid levels in the blood — in adults with gout. It is specifically for people who have not responded adequately to the highest tolerated dose of allopurinol, who cannot tolerate allopurinol, or for whom allopurinol is not appropriate.
Important limitation: febuxostat is not recommended for people who have high uric acid but no gout symptoms (asymptomatic hyperuricemia). It is also not recommended for patients whose high uric acid is caused by other conditions such as cancer treatment or Lesch-Nyhan syndrome.
