Niacin has been associated with cases of liver toxicity, especially when used in pharmacologic doses. Sustained-release niacin preparations appear to be associated with a higher risk of hepatotoxicity than immediate-release niacin.

Here's the gist with niacin and nitrofurantoin: both can occasionally be hard on your liver. Nitrofurantoin, while uncommon, has been linked to liver inflammation, and high, drug-level doses of niacin can do the same. The worry is that stacking two liver stressors together could add up. This isn't about niacin changing how the antibiotic works, it's simply two things that share a similar side effect.

I'll be honest, this is a theoretical concern based on how each is processed, not something proven to happen often. Everyday vitamin amounts of niacin from food or a multivitamin are very unlikely to matter. It's really the high-dose, especially sustained-release, niacin to watch. Since nitrofurantoin is usually a short course, this is rarely a big deal, but check with your pharmacist or doctor if you take prescription-strength niacin.

About Nitrofurantoin

What Is Nitrofurantoin Used For?

Brand names: Macrodantin, Furadantin, Furatoin, Macrobid

Nitrofurantoin treats urinary tract infections (UTIs) caused by specific susceptible bacteria. The capsule forms (including Macrodantin and nitrofurantoin macrocrystals capsules) are indicated for UTIs due to Escherichia coli, enterococci, Staphylococcus aureus, and certain strains of Klebsiella and Enterobacter species. Nitrofurantoin capsules (monohydrate/macrocrystals) are indicated only for acute uncomplicated UTIs (acute cystitis) caused by E. coli or Staphylococcus saprophyticus.

Nitrofurantoin oral suspension (Furadantin) is indicated for adults and children 1 month of age and older for UTIs caused by susceptible strains of E. coli, Enterococcus species, S. aureus, Klebsiella species, and Enterobacter species. No form of nitrofurantoin is approved for kidney infections (pyelonephritis) or infections in the tissue surrounding the kidneys (perinephric abscesses).
