Theoretically, concomitant use of niacin and hepatotoxic drugs might increase the risk of hepatotoxicity.
Niacin has been associated with cases of liver toxicity, especially when used in pharmacologic doses. Sustained-release niacin preparations appear to be associated with a higher risk of hepatotoxicity than immediate-release niacin.

Here's the gist with niacin (vitamin B3) and glyburide. This isn't a blood sugar tug-of-war or a metabolism problem. The concern is the liver. High, drug-like doses of niacin (the cholesterol kind, not the small amounts in food or a basic multivitamin) can occasionally stress the liver. Glyburide, like many medications, is also processed by the liver and carries its own small liver risk. Stacking the two could theoretically add up.

To be honest, this is a theoretical caution based on how these work, not strong proof, so I don't want you to panic. Everyday food amounts of niacin won't matter. If you take high-dose or sustained-release niacin (which is harder on the liver), just loop in your doctor or pharmacist. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and don't change anything on your own.

Glyburide also falls under ANTIDIABETES DRUGS.

Niacin can increase blood glucose levels and may diminish the effects of antidiabetes drugs.
Niacin impairs glucose tolerance in a dose-dependent manner, probably by causing or aggravating insulin resistance and increasing hepatic production of glucose. In diabetes patients, niacin 4.5 grams daily for 5 weeks can increase plasma glucose by an average of 16% and glycated hemoglobin (HbA1c) by 21%. However, lower doses of 1.5 grams daily or less appear to have minimal effects on blood glucose. In some patients, glucose levels increase when niacin is started, but then return to baseline when a stable dose is reached. Up to 35% of patients with diabetes may need adjustments in hypoglycemic therapy when niacin is added.

Here's the thing to know if you take glyburide for type 2 diabetes and are thinking about a niacin supplement: high doses of niacin can actually nudge your blood sugar upward. It does this by making your body a little more resistant to insulin and prompting your liver to release more glucose. That can work against what glyburide is trying to do, so your numbers may creep higher than usual.

The size of the dose really matters. Big drug-like doses (around 1.5 grams a day or more) are where this shows up, while the small amounts in a regular multivitamin or in food are very unlikely to matter. Some people see a bump when starting niacin that settles once they're on a steady dose. If you're starting niacin, keep a closer eye on your glucose readings and check in with your pharmacist or doctor before changing anything.

What Is Glyburide Used For?

Brand names: Diabeta, Micronase, Glynase, Glycron, Micronized, Glynase PresTab, Albert Glyburide

Glyburide tablets are used alongside diet and exercise to help control blood sugar in adults with type 2 diabetes. It is not for type 1 diabetes or diabetic ketoacidosis (a serious complication where the body produces dangerous levels of acids called ketones).

Glyburide may be used alone or combined with metformin when metformin alone is not enough. It can also be used to help transition patients who are moving from insulin or other diabetes pills to a new regimen, under a doctor's guidance.
