Alteplase, tPA falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Plum interacts with.

Theoretically, plum juice might have antiplatelet effects.
Consuming plum juice while taking anticoagulant or antiplatelet drugs might increase the risk of bruising and bleeding. In healthy volunteers, drinking plum juice 200 mL daily for 28 days prolonged clotting time and inhibited platelet aggregation.

Here's something to keep in mind: Alteplase (tPA) is a powerful clot-busting medicine given in the hospital during emergencies like a stroke, heart attack, or a serious blood clot. Its whole job is to break down clots, so anything that adds to bleeding risk matters. One small study found that drinking about 200 mL of plum juice daily for 28 days made blood take longer to clot and slowed platelets a bit.

So in theory, lots of plum juice could nudge bleeding risk slightly higher. That said, alteplase is given by trained staff who watch you closely, and eating a normal amount of plums or prunes as food is very unlikely to cause a problem. If you regularly drink concentrated plum juice, just mention it to your care team.
