Here is the interaction we found between these two, with the evidence behind it.
WARFARIN (Coumadin)
L-carnitine, the parent compound of propionyl-L-carnitine, might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with propionyl-L-carnitine and warfarin.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture: there's a theoretical concern that propionyl-L-carnitine could nudge warfarin to work a little harder than intended. This idea comes mostly from a report involving plain L-carnitine (the parent compound) and a warfarin-like drug used in Europe. We don't have solid proof it happens with propionyl-L-carnitine and warfarin specifically, so think of this as a 'keep an eye out,' not a red alert.
If warfarin's effect did get stronger, your INR could drift up, raising the risk of bleeding, things like easy bruising, nosebleeds, bleeding gums, or blood in the urine or stool. Because warfarin is so sensitive to small changes, please don't start this supplement without telling your pharmacist or doctor first. They may simply check your INR a bit sooner to be safe.

About Warfarin
What Is Warfarin Used For?
Brand name: Warfarin
Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.
In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
