Cilostazol falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Wasabi interacts with.
In vitro research shows that 6-(methylsulfinyl) hexyl isothiocyanate, a constituent of wasabi, inhibits platelet aggregation.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: cilostazol (Pletal) already works by making your platelets less sticky so blood flows more easily to your legs. Wasabi contains a natural compound that, in lab dish studies, also slowed platelets from clumping. Stack the two and, in theory, you could nudge yourself toward easier bruising, nosebleeds, bleeding gums, or blood that takes longer to stop.
The reassuring part: this concern is purely theoretical and comes from test-tube work, not from people. A normal dab of wasabi on your sushi is very unlikely to cause any trouble. The question mark is really about concentrated wasabi supplements taken daily. Before starting one, run it by your pharmacist or doctor, and let them know if you notice any unusual bleeding.
