Wasabi and Clopidogrel

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Clopidogrel

Clopidogrel falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Wasabi interacts with.

Theoretically, wasabi might increase the risk of bleeding with anticoagulant or antiplatelet drugs.
In vitro research shows that 6-(methylsulfinyl) hexyl isothiocyanate, a constituent of wasabi, inhibits platelet aggregation.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: clopidogrel (Plavix) already works by keeping your platelets from clumping together, which lowers your risk of clots. Some lab studies suggest a compound in wasabi can also gently quiet down platelet activity. So in theory, the two together could nudge your bleeding risk up a little, meaning easier bruising, longer bleeding from small cuts, or nosebleeds.

I want to keep this in perspective, though. This concern comes only from test-tube research, not from studies in people, and the small dab of wasabi you'd eat with sushi is very unlikely to cause a problem. Concentrated wasabi supplements are a different story. Since clopidogrel is important heart medicine, just run any wasabi supplement past your pharmacist or doctor first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Wasabi

What is Wasabi?

Wasabi
  • Flavoring for sushi and other foods
  • Antimicrobial support
  • Anti-inflammatory support
  • Antioxidant support
  • General immune support

Wasabi is best known as the pungent green condiment served with sushi, made from a Japanese plant in the cabbage family. Its natural compounds (isothiocyanates) show antimicrobial and antioxidant activity in lab studies, but there is very little human research to support taking it as a supplement. As a food it is generally safe; as a concentrated supplement, talk with your pharmacist or doctor first.

About Clopidogrel

What Is Clopidogrel Used For?

Brand name: Plavix

Clopidogrel is used to lower the risk of heart attack and stroke in people whose arteries are already affected by disease.

Its supported uses are acute coronary syndrome (including unstable angina, non-ST-elevation heart attack and ST-elevation heart attack), a recent heart attack, a recent stroke, and established peripheral arterial disease (narrowed arteries, usually in the legs). For acute coronary syndrome, some product labels say it should be used together with aspirin.
