Palifermin falls under ORAL DRUGS — the group Xanthan Gum interacts with.

Theoretically, xanthan gum can alter the absorption of oral drugs due to its fiber qualities. Xanthan gum slows gastric emptying and has been used to control the release of drugs in tablet formulations. To avoid any alterations in drug absorption, xanthan gum should be taken 30-60 minutes after oral medications.

Here's some reassuring news: this concern really doesn't apply to palifermin (Kepivance). The interaction on file is about xanthan gum's fiber gel possibly slowing how fast oral medications get absorbed from the gut. But palifermin isn't a pill you swallow. It's given as an injection into a vein by your care team, so it never travels through your stomach or intestines where xanthan gum does its thickening work.

Because of that, there's no real overlap here. This was only a theoretical, lab-based idea about oral tablets, and palifermin doesn't fit that picture. If you're using xanthan gum to thicken liquids during chemo, that's a smart conversation to have with your pharmacist or oncology team, but it shouldn't change how palifermin works.

It is generally recognized as safe in the small amounts found in food, but large doses can cause gas, bloating, and loose stools. About Palifermin

What Is Palifermin Used For?

Brand name: Kepivance

Kepivance (palifermin) is used to decrease how often and how long severe oral mucositis — painful sores in the mouth and throat — occurs in patients with hematologic malignancies (blood cancers such as lymphoma, leukemia, or multiple myeloma) who are receiving high-dose, cell-damaging (myelotoxic) therapy before an autologous hematopoietic stem cell transplant (a transplant using the patient's own stem cells).

Kepivance is specifically indicated when the preparative chemotherapy or radiation regimen is expected to cause severe mucositis in most patients. It is not recommended for use with a high-dose melphalan conditioning regimen, and it was not found to be effective in the setting of allogeneic stem cell transplant (using a donor's cells). Its safety and effectiveness have not been established for patients with non-blood cancers (non-hematologic malignancies).
