Overview

Betaine anhydrous, also known as trimethylglycine (TMG), is a compound that occurs naturally in the body and in many foods. Good food sources include sugar beets, regular beets, spinach, wheat bran, and whole grains. The supplement is often made from sugar beets or produced synthetically.

Betaine has two main uses. As a prescription medicine, betaine anhydrous is approved to help treat a rare inherited disorder called homocystinuria, which causes very high levels of an amino acid called homocysteine. As a dietary supplement, it is popular in sports nutrition and is marketed for liver health and heart health.

It is important not to confuse betaine anhydrous (TMG) with betaine hydrochloride (betaine HCl), a related product used to add acid to the stomach for digestion. They are different supplements.

How it works

Betaine acts as a methyl donor. This means it can give away part of its chemical structure (a methyl group) to help convert homocysteine into another amino acid called methionine. By doing this, it helps lower homocysteine levels in the blood.

Betaine also works as an osmolyte, a molecule that helps cells balance water and protect themselves from stress. Researchers think this role may be one reason it is studied for exercise performance and cell health.

Many of the proposed benefits beyond homocysteine lowering are based on laboratory and animal research or small human studies, so the mechanisms are better understood than the real-world benefits.

Safety & precautions

Betaine anhydrous appears to be generally well tolerated in the doses used in studies and in prescription form. However, people with certain conditions should be cautious. One concern is that betaine can raise blood levels of cholesterol (including LDL) in some people, which may matter for those with heart disease risk factors.

People with kidney disease or other chronic health conditions should talk to a healthcare provider before using betaine.

For pregnancy, there is not enough reliable safety information, so it is best to avoid betaine supplements unless your doctor specifically recommends them. For breastfeeding, safety has not been well studied, so caution is advised and you should check with your provider first.

As with any supplement, choose products from reputable brands, since supplements are not as tightly regulated as medicines.

Side effects

Betaine is usually well tolerated. The most common side effects are digestive, such as nausea, stomach upset, diarrhea, or a noticeable body odor.

A more serious but rare concern, mostly reported with high-dose prescription use in people with homocystinuria, is brain swelling (cerebral edema). This is uncommon, but anyone using prescription betaine should be monitored by their doctor.

Stop using betaine and contact a healthcare provider if you have severe or unusual symptoms such as confusion, severe headache, vomiting, or vision changes.

When used orally to treat homocystinuria due to cystathionine beta-synthase deficiency, elevated plasma methionine concentrations can occur following use of betaine anhydrous, which might lead to cerebral edema.

Myasthenia gravis Cardiovascular

Betaine anhydrous might have adverse effects on the plasma lipid profile. Some studies have reported a 3% to 4% increase in total and low-density lipoprotein (LDL) cholesterol levels with betaine anhydrous 6 grams daily. A meta-analysis of 6 studies in adults, some with obesity and/or prediabetes, shows that taking betaine anhydrous 4-6 grams daily for 6-24 weeks is associated with a mean increase in total cholesterol of 4 mg/dL, with no significant change in LDL cholesterol, high-density lipoprotein (HDL) cholesterol, or triglyceride levels. Another meta-analysis of 12 studies, some in healthy adults and others in adults with various disease states, shows that taking betaine anhydrous 1.5-20 grams daily for 2-52 weeks is associated with a mean increase in total cholesterol of 14 mg/dL, and a mean increase in LDL cholesterol of 10 mg/dL, with no change in triglyceride or HDL cholesterol levels.

Dosing

There is no single standardized dose for betaine anhydrous as a general supplement. Doses used in sports nutrition studies and other research vary widely.

The prescription form used for homocystinuria is dosed by a doctor based on the patient's needs and lab results, and should only be used under medical supervision.
