The evidence for shiitake mushroom in this product is mixed and mostly limited. For dental plaque, it's rated possibly ineffective.
For cancer, chemotherapy-related anemia, chemotherapy-related nausea and vomiting, atherosclerosis, and breast cancer, we don't have enough reliable evidence to rate how well it works. How safe is it?

Shiitake mushroom eaten as food is generally well tolerated. In supplement form at concentrated doses, though, the safety data is thin, so caution is advised.
When taken orally, some people report abdominal discomfort, bloating, diarrhea, nausea, and vomiting. Rarely, raw shiitake can cause a skin eruption called shiitake dermatitis, and large pieces that aren't chewed well can cause intestinal blockage.
People who handle shiitake mushrooms extensively may develop allergic contact dermatitis or, in rare cases, breathing problems from spore inhalation.

Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Immunosuppressants
Theoretically, taking shiitake mushroom might decrease the effects of immunosuppressive therapy.
In vitro evidence suggests that shiitake mushroom extracts stimulate immune function.
