Alfalfa leaf is generally well tolerated when taken in small amounts as food, but supplement doses are another story. The most common side effects are gastrointestinal: abdominal discomfort, diarrhea, intestinal gas, and bulkier stools — sometimes noticeable in the first week.

A lupus-like syndrome has been reported rarely after long-term use. Dermatitis (skin inflammation) has also been reported, including a case where it developed after two months of regular alfalfa tea and improved when the person stopped.

For pregnancy and breastfeeding, the safety data advises against this supplement. Alfalfa has estrogen-like activity and is best avoided in supplement amounts during pregnancy.
