Thiamine is generally very well tolerated by mouth at normal doses. Allergic skin reactions and other allergic reactions are rare.

Thiamine is needed in pregnancy and while breastfeeding, and the safety rating is Likely Safe — but use only the amounts your doctor advises. Benfotiamine is generally well tolerated in the studies we have, but long-term safety hasn't been fully studied.

In one trial of 98 people taking 300–600 mg daily for 6 weeks, a small number reported allergic skin reactions and slight stomach discomfort, at rates no different from placebo. We don't have enough data on benfotiamine safety during pregnancy or breastfeeding, so check with your doctor or pharmacist before taking it if you're pregnant or nursing.

Trimethoprim might increase blood levels of thiamine.
In vitro, animal, and clinical research suggest that trimethoprim inhibits intestinal thiamine transporter ThTR-2, hepatic transporter OCT1, and renal transporters OCT2, MATE1, and MATE2, resulting in paradoxically increased thiamine plasma concentrations.
