Peony is generally well tolerated when used short-term, though high-quality safety data are limited. The most common side effects when taken by mouth are mild digestive complaints: abdominal bloating, loss of appetite, diarrhea, nausea, and general gut discomfort.

In one clinical trial, diarrhea occurred in 5% of people taking a peony extract at a fairly high dose for 24 weeks, compared to 1% on placebo. If you apply peony topically, skin irritation (dermatitis) is possible.

Do not use this product during pregnancy—there isn't enough safety data, and some traditional uses raise concern. There is also not enough information to consider it safe while breastfeeding.
