The evidence supports coQ-10 for coenzyme Q10 deficiency—that's rated likely effective. For other uses, the data is more limited.
It's rated possibly effective for fibromyalgia, migraine headaches, congestive heart failure, and diabetic neuropathy (nerve damage from diabetes), but "possibly effective" means the evidence is promising but not yet conclusive. If you're considering coQ-10 for one of these conditions, talk with your doctor about whether it makes sense for you.
How safe is it?
Well-documented data
CoQ-10 is generally well tolerated by most adults. In clinical studies, no serious adverse effects have been reported.
The most common side effects are gastrointestinal—appetite suppression, diarrhea, heartburn, nausea, or vomiting—but these occur in less than 1% of people. Headache, dizziness, and insomnia have been reported rarely; dividing doses above 100 mg daily may help reduce stomach upset.
During pregnancy, there isn't enough reliable safety data, so it's best to avoid it unless your doctor advises otherwise. Safety while breastfeeding hasn't been well studied either, so check with your doctor before use if you're nursing.

The most serious concern is with chemotherapy drugs called alkylating agents (such as cyclophosphamide). CoQ-10's antioxidant effects may theoretically reduce how well these drugs work by protecting tumor cells from the oxidative stress these chemotherapies rely on to kill cancer cells.

CoQ-10 also interacts with warfarin (Coumadin), a blood thinner. Because coQ-10 is chemically similar to vitamin K, it may have blood-clotting effects that reduce warfarin's effectiveness.
Four cases of decreased warfarin efficacy have been reported in association with coQ-10 use.
Finally, coQ-10 may lower blood pressure on its own. If you take medications to manage high blood pressure (antihypertensive drugs), adding coQ-10 could theoretically add to that effect, though this is a minor concern based on the data.

Coenzyme Q10
Alkylating Agents
Coenzyme Q10 has antioxidant effects. Theoretically, this may reduce the activity of chemotherapy drugs that generate free radicals.
Theoretically, antioxidants such as coenzyme Q10 might protect tumor cells from chemotherapeutic agents that work by inducing oxidative stress, such as alkylating agents (e.g., cyclophosphamide) and radiation therapy. The clinical importance of this interaction is unknown.
Warfarin (Coumadin)
Coenzyme Q10 is chemically similar to menaquinone and might have vitamin K-like procoagulant effects, which could decrease the effects of warfarin.
Concomitant use of coenzyme Q10 and warfarin might reduce the anticoagulant effects of warfarin. Four cases of decreased warfarin efficacy thought to be due to coenzyme Q10 have been reported. However, there is some preliminary clinical research that suggests coenzyme Q10 might not significantly decrease the effects of warfarin in patients who have a stable INR.
Antihypertensive Drugs
Theoretically, coenzyme Q10 might have additive effects with antihypertensive drugs.
Some clinical research shows that coenzyme Q10 can significantly lower blood pressure, although other studies have shown conflicting results.
