Does it work?
Not established
The evidence for what these mushrooms actually do is thin. For cordyceps, the data shows it's possibly ineffective for athletic performance, and there isn't enough reliable evidence to rate it for sexual dysfunction, irregular heart rhythm, asthma, lung cancer, or chronic lung disease.
Reishi appears possibly ineffective for high cholesterol, and there's insufficient evidence for altitude sickness, Alzheimer's disease, or athletic performance. Shiitake is possibly ineffective for dental plaque and has insufficient evidence for chemotherapy-related anemia, artery hardening, breast cancer, cancer in general, or chemotherapy nausea and vomiting.

How safe is it?
Well-documented data
All three mushrooms are generally well tolerated short-term. With cordyceps, the most common side effects are belly discomfort, constipation, and diarrhea — usually mild and sometimes reduced by taking it with food.
It's been studied for up to a year with no major red flags, though quality varies widely between products. One rare case reported liver inflammation (hepatitis) possibly linked to cordyceps, but it's unclear whether cordyceps or a contaminant caused it.
Reishi commonly causes dizziness, dry mouth, itching, nausea, rash, and stomach upset. Rarely, it may cause insomnia or headache.
A few people have had allergy symptoms like sore throat or runny nose from reishi spores. Shiitake can cause belly discomfort, bloating, diarrhea, nausea, and vomiting when taken as a concentrated supplement.
Eating shiitake as food is generally fine, but raw shiitake can cause a rash (shiitake dermatitis) with whiplash-like marks, and in rare cases, large pieces can block the intestines. People who handle shiitake extensively may develop allergies.
There isn't enough safety data on any of these mushrooms during pregnancy or breastfeeding, so the safety data advises against their use in both situations. Talk with your doctor or pharmacist about whether this product is right for you.

Reishi Mushroom Fruiting Body Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, high doses of reishi mushroom might increase the risk of bleeding.
A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.
Antidiabetes Drugs
Theoretically, reishi mushroom might have additive effects with antidiabetes drugs.
Animal research suggests that reishi mushroom decreases blood sugar. However, in patients with type 2 diabetes, taking reishi mushroom does not reduce fasting glucose levels, and its effects on glycated hemoglobin are inconsistent.
Antihypertensive Drugs
Theoretically, concurrent use of reishi mushroom with antihypertensive drugs might increase the risk of hypotension.
Reishi mushroom has shown hypotensive activity in animal research. Clinical evidence suggests that reishi mushroom reduces blood pressure in some, but not all, patients with hypertension.
Shiitake Mushroom Fruiting Body Extract
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Immunosuppressants
Theoretically, taking shiitake mushroom might decrease the effects of immunosuppressive therapy.
In vitro evidence suggests that shiitake mushroom extracts stimulate immune function.
Cordyceps sinensis Mushroom Mycelium Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, cordyceps may increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
In vitro and animal research suggests that cordyceps extract inhibits platelet aggregation and function. However, this interaction has not been reported in humans.
Immunosuppressants
Theoretically, concurrent use of cordyceps might interfere with immunosuppressive therapy.
Animal and in vitro research suggests that cordyceps stimulates the immune system. However, limited clinical research suggests that taking cordyceps may lower the necessary therapeutic dose of the immunosuppressant cyclosporine, which suggests that cordyceps may have an immunosuppressive effect.
Testosterone
Theoretically, concurrent use of cordyceps and testosterone might have additive effects.
Animal research suggests that cordyceps can increase testosterone levels. The clinical significance of this finding is unclear.
