Nattokinase is generally well tolerated over the short term, but because it thins the blood, it carries risk if you have bleeding disorders or a history of bleeding. Rare reports include allergic reactions (even anaphylaxis) in people sensitive to natto, with symptoms appearing within 2 hours of taking it.

Boswellia serrata is generally well tolerated short-term; common side effects are mild — abdominal pain, diarrhea, headache, heartburn, nausea, and dizziness. Rarely, very large amounts can form a bezoar (a mass in the stomach).

For pregnancy: nattokinase is not recommended — not enough safety data exists. Boswellia serrata carries insufficient data as well, so do not take it while pregnant or breastfeeding without talking to your doctor or pharmacist.

However, other in vitro research suggests that, when coupled with calcium ions, boswellic acids containing the keto group have immunostimulant properties within specific cell signaling pathways.
