Acetaminophen, Chlorzoxazone, Codeine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Boswellia Serrata interacts with.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2D6 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill contains codeine, which is the part that matters here. Codeine is a prodrug, meaning your body has to convert it using an enzyme called CYP2D6 before it can relieve pain. Lab studies suggest boswellia may slow that same enzyme down. If that happened in real life, less codeine would turn into its active form, so you might get weaker pain relief rather than more side effects.
Honestly, this concern is theoretical and comes only from test-tube research, not from people, so I wouldn't lose sleep over it. The acetaminophen and chlorzoxazone parts aren't really affected. If you take this regularly for pain, just mention the boswellia to your pharmacist or doctor so they can keep an eye on how well it's working.

This combination pill pairs acetaminophen and codeine for pain with chlorzoxazone, a muscle relaxer. The piece that touches on this interaction is the chlorzoxazone, which the body partly clears using a liver enzyme called CYP1A2. Lab (test tube) studies hint that boswellic acids in Boswellia can slow down CYP1A2. If that happened in real life, chlorzoxazone could build up a bit and lead to more drowsiness, dizziness, or an upset stomach.
I want to be honest though: this is theoretical, based only on early lab work, not on real patients. So I wouldn't lose sleep over it. Just let your pharmacist or doctor know you're adding Boswellia, especially since this medicine already makes many people sleepy.
