Does it work?
Not established
The evidence on these ingredients is mixed. For D-Ribose, clinical trials haven't shown it reliably helps athletic performance, McArdle disease, or muscle soreness — those ratings are either possibly ineffective or have insufficient evidence.
Evidence is also lacking for restless legs syndrome, congestive heart failure, and recovery after heart bypass surgery. Peak ATP has an 'Effective' rating for treating paroxysmal supraventricular tachycardia (a type of irregular heartbeat) and cardiovascular disease diagnosis, but those uses are established in medical settings, not in over-the-counter supplements.
How safe is it?
Well-documented data
D-Ribose is generally well tolerated over short periods — up to about a month — but long-term safety hasn't been well studied. It can lower blood sugar, which is important if you're already at risk for low blood sugar.
The most common side effects when taken by mouth are diarrhea, stomach discomfort, headache, nausea, and low blood sugar itself. Some people report anxiety, lightheadedness, or increased appetite; lowering the dose has resolved these in studies.
Peak ATP (adenosine) as an oral supplement has very limited safety data; the prescription injectable form is potent and must be given only by medical professionals. There is not enough information to know if either ingredient is safe in pregnancy or while breastfeeding — talk with your doctor or pharmacist about this before taking it if you're pregnant, planning pregnancy, or nursing.

D-Ribose
Antidiabetes Drugs
Theoretically, taking ribose in combination with antidiabetes drugs might increase the risk of hypoglycemia.
In clinical research, ribose decreases serum glucose levels in a dose-dependent manner.
Insulin
Theoretically, taking ribose with insulin could increase the hypoglycemic effect of insulin.
In clinical pharmacokinetic studies, oral administration of ribose modestly increased serum insulin levels.
Peak ATP
Dipyridamole (Persantine)
Dipyridamole can increase the therapeutic and toxic effects of adenosine.
Dipyridamole decreases the metabolism of adenosine. Intravenous infusion of adenosine in patients who are taking dipyridamole can cause dizziness, bradycardia, and syncope. Dipyridamole should be discontinued for several days prior to a cardiac stress test using adenosine.
Carbamazepine (Tegretol)
Carbamazepine might increase the risk of heart block when used concomitantly with adenosine.
Carbamazepine and adenosine can both cause heart block. Giving them concurrently might produce an additive effect.
Methylxanthines
Methylxanthines are competitive antagonists of adenosine and can block its pharmacologic effects.
The methylxanthines, aminophylline, caffeine, and theophylline, can block the effects of adenosine by acting as competitive antagonists at adenosine cell surface receptors. It is recommended that methylxanthines be avoided for 24 hours prior to cardiac stress tests.
