Most of these enzymes are generally well tolerated when taken as directed. The main concern is allergic reactions — papain and bromelain can trigger itching, swelling, rashes, or respiratory symptoms in sensitive people.

Papain may also cause stomach irritation or, rarely at high doses, serious damage to the esophagus or stomach lining. Bromelain can cause diarrhea, gas, or stomach upset.
