Feverfew is generally well tolerated short-term for most people, though it can cause allergic reactions and mouth irritation in some. The most common side effects when taken by mouth are gastrointestinal: abdominal pain, bloating, constipation, diarrhea, flatulence, heartburn, and nausea.

A skin rash may appear in sensitive individuals. Chewing fresh leaves is more likely to cause mouth ulceration and numbness than capsules.

One important note: if you've taken feverfew long-term and then stop, you may experience "post-feverfew syndrome" — anxiety, headaches, and insomnia — so taper off gradually rather than quitting cold. Avoid feverfew during pregnancy, as it may stimulate the uterus and its safety isn't established.

Anticoagulant/Antiplatelet Drugs

Theoretically, feverfew might have additive effects and increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Laboratory research suggests that feverfew may inhibit platelet aggregation. Additionally, in one case report, a 36-year-old patient taking feverfew 2400 mg daily for 3 months experienced vaginal bleeding and a prolonged menstrual cycle, with a modest increase in partial thromboplastin time (PTT) and prothrombin time (PT).
