Flex-Ability is a liquid herbal blend containing 11 active ingredients, most rooted in traditional Asian medicine. The two with the most data are Dong Quai and Lycium (goji berry), along with Achyranthes, Uncaria, Siler, Ligusticum chuanxiong, Flowering Quince, Notopterygium, Sichuan Teasel, and Tienchi Ginseng.
The product also contains purified water and grain alcohol as inactive ingredients.

The evidence we hold does not establish effectiveness for any condition. Dong Quai has been studied for iron deficiency anemia, osteoporosis, coronary heart disease, migraine headache, and eczema, but the evidence is insufficient to rate it.
Lycium (goji) shows the same picture for sunburn, cardiovascular disease, high blood pressure, obesity, and quality of life. For the other nine ingredients in this blend, we have no effectiveness ratings on file.
How safe is it?
Well-documented data
Dong Quai is generally well tolerated when taken by mouth, though some people report burping and flatulence. It can increase your risk of bleeding and make your skin more sensitive to sunlight, so use caution.
The safety data advises against taking it during pregnancy because it may stimulate the uterus and affect bleeding. There isn't enough information to know if it's safe while breastfeeding.
Lycium (goji) is generally safe as food but has less safety data in concentrated supplement form. Avoid medicinal amounts during pregnancy because of limited data and traditional concerns about effects on the uterus.
While breastfeeding, there isn't enough reliable data, so caution is advised. Rarely, people have reported allergic reactions including swelling of the face, throat tightness, and rash, and one case of anaphylaxis has been reported.
A small number of cases have also reported liver injury, though whether goji was the cause is unclear.

Also verify aspirin and other antiplatelet drugs (Moderate severity for bleeding risk) and any estrogen-based hormones like birth control (Moderate severity).

Flex-Ability by Planetary Herbals Drug Interactions

Flex-Ability by Planetary Herbals contains two ingredients with documented interactions: Dong Quai and Lycium (goji).
The most serious concern is with blood thinners (anticoagulants). Dong Quai carries a Major interaction with warfarin (Coumadin)—case reports show it can increase warfarin's effect and raise bleeding risk.
Lycium also has a Major interaction with warfarin, documented in at least 5 cases where people drinking concentrated goji juice or tea had increased bleeding markers after starting the supplement.

Lycium also interacts moderately with several other drug types. It may theoretically reduce how your body metabolizes drugs broken down by cytochrome P450 enzymes (CYP2D6, CYP2C19, CYP2C9, and CYP3A4 substrates)—meaning levels of those drugs could rise.
Additionally, it carries a Moderate interaction with heart rhythm medications like flecainide (Tambocor) and blood pressure drugs, and a Minor interaction with diabetes medications.
Dong Quai has a Moderate interaction with estrogen-based hormones (like birth control or hormone replacement therapy) and with antiplatelet drugs such as aspirin or clopidogrel (Plavix), where it may increase bleeding risk.

Lycium
Warfarin (Coumadin)
Goji can increase the effects of warfarin and possibly increase the risk of bleeding.
There are at least 5 case reports of increased international normalized ratio (INR) in patients stabilized on warfarin who began drinking goji juice, concentrated goji tea, or goji wine. Goji may inhibit the metabolism of warfarin by cytochrome P450 2C9 (CYP2C9).
Antihypertensive Drugs
Theoretically, concomitant use of goji root bark, but not goji fruit, with antihypertensive drugs might have additive effects.
Animal and in vitro research suggest that goji root bark has hypotensive effects. However, goji fruit juice does not appear to reduce systolic or diastolic blood pressure in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, goji berry might inhibit CYP2C19 and reduce metabolism of CYP2C19 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C19 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2C19 substrates. However, this has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, goji berry might inhibit CYP2C9 and reduce metabolism of CYP2C9 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C9 enzymes. Additionally, multiple case reports suggest that goji berry concentrated tea and juice inhibit the metabolism of warfarin, a CYP2C9 substrate. Concomitant use with goji may decrease metabolism and increase levels of CYP2C9 substrates.
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, goji berry might inhibit CYP2D6 and reduce metabolism of CYP2D6 substrates.
In vitro research shows that goji berry juice inhibits CYP2D6 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2D6 substrates. However, this has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Flecainide (Tambocor)
Theoretically, goji berry might increase the levels and clinical effects of flecainide.
In one case report, a 75-year-old patient stable on flecainide and warfarin presented to the emergency room with fainting and pleomorphic arrhythmia caused by flecainide toxicity. Flecainide toxicity was attributed to drinking 1-2 glasses of concentrated goji tea daily for 2 weeks. Theoretically, goji may have inhibited the cytochrome P450 2D6 (CYP2D6) metabolism of flecainide.
Antidiabetes Drugs
Theoretically, concomitant use of goji fruit polysaccharides or goji root bark with antidiabetes drugs might have additive effects.
Animal and in vitro research show that goji root bark and fruit polysaccharides might have hypoglycemic effects. However, clinical research has only shown that taking goji fruit polysaccharides with or without antidiabetes drugs modestly reduces postprandial glucose when compared with control, with no reports of hypoglycemia.
Tienchi Ginseng
Aspirin
Theoretically, taking Panax notoginseng concomitantly with aspirin may increase the risk of adverse effects from both products.
Animal research shows that taking Panax notoginseng extract with aspirin increases blood levels of salicylic acid by approximately 50% and blood levels of Panax notoginseng by 75% to 196%. This effect may be due to increased absorption of both products.
Caffeine
Theoretically, taking Panax notoginseng may decrease the levels and clinical effects of caffeine.
Animal research shows that administering Panax notoginseng intravenously for 7 days before intraperitoneal injection of caffeine can decrease maximal blood levels of caffeine by 37%. This interaction is attributed to the ability of Panax notoginseng to increase the activity of cytochrome P450 1A2 (CYP1A2) enzymes.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, taking Panax notoginseng might reduce the levels and clinical effects of CYP1A2 substrates.
Animal research shows that administering Panax notoginseng intravenously for 7 days before intraperitoneal injection of caffeine can decrease maximal blood levels of caffeine by 37%. This interaction was attributed to the ability of Panax notoginseng to increase the activity of CYP1A2.
Warfarin (Coumadin)
Theoretically, taking Panax notoginseng concomitantly with warfarin may increase the risk of bleeding.
Animal research shows that taking Panax notoginseng concomitantly with warfarin increases plasma warfarin levels, prothrombin time, and international normalized ratio when compared with control. In vitro research also suggests that Panax notoginseng may downregulate expression of cytochrome P450 3A4 enzymes, which may affect warfarin metabolism.
Dong Quai
Warfarin (Coumadin)
Dong quai may increase the risk of bleeding when used with warfarin.
Case reports suggest that concomitant use of dong quai with warfarin can increase the anticoagulant effects of warfarin and increase the risk of bleeding. In one case, after 4 weeks of taking dong quai 565 mg once or twice daily, the international normalized ratio (INR) increased to 4.9. The INR normalized 4 weeks after discontinuation of dong quai.
Anticoagulant/Antiplatelet Drugs
Theoretically, dong quai may increase the risk of bleeding when used with anticoagulant or antiplatelet drugs; however, research is conflicting.
Animal studies suggest that dong quai has antithrombin activity and inhibits platelet aggregation due to its coumarin components. Additionally, some case reports in humans suggest that dong quai can increase the anticoagulant effects of warfarin. However, clinical research in healthy adults shows that taking 1 gram of dong quai root daily for 3 weeks does not significantly inhibit platelet aggregation or cause bleeding. Until more is known, use dong quai with caution in patients taking antiplatelet/anticoagulant drugs.
Estrogens
Theoretically, dong quai may reduce the effects of estrogens.
Dong quai has estrogenic effects. Theoretically, concomitant use of large amounts of dong quai might interfere with hormone replacement therapy due to competition for estrogen receptors.
