Goji and Acetaminophen, Codeine, Doxylamine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Why this applies to Acetaminophen, Codeine, Doxylamine

Acetaminophen, Codeine, Doxylamine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP2D6 and reduce metabolism of CYP2D6 substrates.
In vitro research shows that goji berry juice inhibits CYP2D6 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2D6 substrates. However, this has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the wrinkle with this combo: the codeine in Mersyndol is a prodrug. Your body uses an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves your pain. Lab studies hint that goji berry juice may slow down that same enzyme. If it does, less codeine gets converted to morphine, so you might get weaker pain relief rather than more side effects.

I want to be honest about how thin this evidence is. It comes only from test-tube research and has never been seen in real people, so for most folks eating goji berries as a food, I wouldn't worry. If you're taking concentrated goji supplements regularly, just give your pharmacist or doctor a quick heads-up so they can keep an eye on how well your pain is controlled.
