Why this applies to Acetaminophen, Codeine, Doxylamine

Acetaminophen, Codeine, Doxylamine falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Ginkgo interacts with.

Theoretically, taking ginkgo with drugs that lower the seizure threshold might increase the risk for convulsions.
Ginkgo seeds contain ginkgotoxin. Large amounts of ginkgotoxin can cause neurotoxicity and seizure. Ginkgotoxin is present in much larger amounts in ginkgo seeds than leaves. Ginkgo leaf extract contains trace amounts of ginkgotoxin. The amount of ginkgotoxin in ginkgo leaf and leaf extract seems unlikely to cause toxicity. However, there are anecdotal reports of seizure occurring after use of ginkgo leaf both in patients without a history of seizure disorder and in those with previously well-controlled epilepsy.

Here's the deal with combining ginkgo and Mersyndol (which is acetaminophen, codeine, and the antihistamine doxylamine). The concern isn't about ginkgo changing how your body breaks down these medicines. Instead, ginkgo seeds contain a compound called ginkgotoxin that, in large amounts, can lower the threshold for seizures. Codeine and doxylamine can both nudge that threshold down a little too, so in theory stacking them together might raise the risk of a convulsion.

I want to keep this in perspective. This is based on rare anecdotal reports, and ginkgo leaf extract (what's in most supplements) contains only trace amounts of ginkgotoxin, unlike the seeds. For most people this is very unlikely to be a problem. Still, if you have a history of seizures, please check with your pharmacist or doctor first.

Theoretically, ginkgo might increase levels of drugs metabolized by CYP1A2.
Laboratory research suggests that ginkgo leaf extract can mildly inhibit CYP1A2 enzymes. However, clinical research suggests ginkgo might not affect CYP1A2. Until more is known, use ginkgo cautiously in patients taking drugs metabolized by these enzymes.

Here's the honest version: this concern is mostly theoretical. Ginkgo may very mildly slow down a liver enzyme called CYP1A2 in the test tube, but actual studies in people suggest it probably doesn't change CYP1A2 much at all. None of the three ingredients in Mersyndol (acetaminophen, codeine, and the antihistamine doxylamine) lean heavily on CYP1A2 to be cleared, so a meaningful interaction here is unlikely.

If anything did shift, you'd want to watch for the usual effects of this combo being a bit stronger: extra drowsiness, dizziness, constipation, or feeling foggy from the codeine and doxylamine. I wouldn't lose sleep over it, but since both can cause sedation, it's worth a quick chat with your pharmacist before pairing them, especially if you'll be driving.
