How safe is it?
Well-documented data
Raspberry ketones have stirred case reports of heart palpitations, rapid heartbeat, and elevated blood pressure — the ingredient resembles a stimulant — though human studies are limited. Glucomannan is generally well tolerated when taken with plenty of water, but it can cause bloating, gas, diarrhea, nausea, and rarely, choking or blockage if swallowed dry.
African mango extract is generally tolerated short-term, though one case of hepatitis has been reported; headache and insomnia occurred in studies. Lychee extract can trigger allergic reactions including swelling of the tongue and lips, and has been linked in children to a serious condition called hypoglycemic encephalopathy, though the link is debated.
For pregnancy, the safety data advises against raspberry ketones and African mango — there isn't enough information on glucomannan or lychee extracts in pregnancy, so talk with your doctor. For breastfeeding, avoid raspberry ketones and African mango; glucomannan and lychee lack enough safety data as well.

The most serious concern is a Moderate-severity interaction between raspberry ketones and stimulant drugs — raspberry ketone resembles the stimulant synephrine, and there's risk of heart palpitations, elevated blood pressure, coronary vasospasm, and dangerous irregular heartbeat rhythms.

Raspberry ketones also interact with warfarin (Coumadin), a blood thinner, at Moderate severity — it may increase how fast your body clears warfarin, meaning you might need a higher dose to keep your blood clotting controlled. Glucomannan can decrease how well your body absorbs oral medications (Moderate severity), so separating doses from your pills by at least two hours may help.
African mango extract and lychee extract both carry Moderate-severity interactions with diabetes medications — they may lower blood sugar further and increase the risk of low blood sugar (hypoglycemia). Lychee extract also has a Minor-severity interaction with immunosuppressants, potentially reducing their effectiveness.

Dietary Fiber
Carbamazepine (Tegretol)
Theoretically, black psyllium might reduce the effects of carbamazepine and increase the risk for convulsions.
Theoretically, black psyllium might reduce carbamazepine absorption. A preliminary study using blond psyllium reported decreased carbamazepine bioavailability due to binding of the drug to psyllium, as well as reduction of available fluid in the gut for dissolution of the drug. This interaction may also occur with black psyllium.
Lithium
Theoretically, taking black psyllium at the same time as lithium might reduce lithium absorption.
The fiber in black psyllium might reduce lithium absorption and plasma levels. Some case reports describe a reduction in plasma lithium levels with concomitant administration of blond psyllium. This was reversed when psyllium was stopped. This interaction may also occur with black psyllium.
Metformin (Glucophage)
Theoretically, black psyllium might increase the therapeutic and adverse effects of metformin.
Animal research shows that concurrent consumption of blond psyllium with metformin slows and increases the absorption of metformin. This interaction may also occur with black psyllium. To avoid changes in absorption, take psyllium 30-60 minutes after metformin.
Olanzapine (Zyprexa)
Theoretically, taking black psyllium at the same time as olanzapine might reduce olanzapine absorption.
The fiber in black psyllium might decrease the absorption of olanzapine. A single case report describes a reduction in the effectiveness of olanzapine when it was concomitantly administered with an unspecified type of psyllium 3 grams orally twice daily. This effect was reversed when psyllium was stopped.
Digoxin (Lanoxin)
Theoretically, taking black psyllium at the same time as digoxin might reduce digoxin absorption and decrease digoxin levels.
Psyllium might bind digoxin in the gut. However, some clinical evidence suggests that psyllium does not impact digoxin absorption.
Ethinyl Estradiol
Theoretically, taking black psyllium at the same time as ethinyl estradiol might alter levels of estradiol.
Concurrent use of blond psyllium with ethinyl estradiol results in a slight increase in the extent of ethinyl estradiol absorption and a slower rate of absorption. This is unlikely to be clinically significant.
Oral Drugs
Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin. Although many of these studies evaluated blond psyllium, the fiber content in black psyllium may have similar effects.
LuraLean(R)
Oral Drugs
Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.
Lychee extract
Antidiabetes Drugs
Theoretically, lychee seed might increase the risk of hypoglycemia when used with antidiabetes drugs.
Animal research suggests that an aqueous extract of lychee seed reduces fasting and 2-hour postprandial blood glucose and improves impaired glucose tolerance in rats with type 2 diabetes. Additionally, retrospective research in children has found an association between lychee consumption and increased odds of acute hypoglycemic encephalopathy.
Immunosuppressants
Theoretically, lychee might reduce the effectiveness of immunosuppressant drugs.
In vitro, flavonoids extracted from lychee show immunostimulant effects. So far, this effect has not been reported in humans.
Raspberry Ketones
Stimulant Drugs
Theoretically, raspberry ketone might increase the risk of adverse cardiovascular effects with stimulant drugs.
Structurally, raspberry ketone resembles synephrine, a known stimulant agent. Heart palpitations, elevated blood pressure, coronary vasospasm, pulseless electrical activity arrest, and resistant polymorphic ventricular tachycardia have been reported in patients taking raspberry ketone.
Warfarin (Coumadin)
Theoretically, raspberry ketone might increase warfarin dose requirements.
In one case report, a patient taking warfarin 55 mg per week had a decrease in INR over a period of one month while taking raspberry ketone 250 mg daily. A warfarin dose increase to 70 mg per week was necessary to maintain a therapeutic INR while taking raspberry ketone. The mechanism for this potential interaction is not known.
African Mango (Irvingia gabonensis) extract
Antidiabetes Drugs
Theoretically, taking Irvingia gabonensis with antidiabetes drugs might increase the risk of hypoglycemia in some patients.
Human research shows that Irvingia gabonensis decreases blood sugar in diabetic patients being treated with antidiabetes agents. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Testosterone
Theoretically, taking Irvingia gabonensis with testosterone products drugs might cause additive effects in some patients.
Animal research suggests that Irvingia gabonensis increases testosterone levels. The mechanism of action is unclear. Until more is known, advise patients taking testosterone to avoid using or use Irvingia gabonensis cautiously.
