German chamomile is generally well tolerated by most adults, but it can trigger allergic reactions in people sensitive to ragweed or related plants. Oral use has been reported to cause nausea and mouth burning, and in rare cases severe allergic reactions including anaphylaxis.

Topically, it may cause allergic skin rashes or eczema. One occupational exposure case documented sneezing, itchy and watery eyes, and severe nasal discharge after 10 years of breathing chamomile dust.
