The most serious concern is with CNS depressants (sedatives, sleep aids, anti-anxiety medications, opioids, and alcohol) — gotu kola may add to their sedative effects and increase drowsiness, dizziness, or impaired thinking. Additionally, gotu kola may have additive harmful effects when combined with hepatotoxic drugs (medications that can damage the liver).
There are at least four case reports of liver toxicity linked to gotu kola use, though whether gotu kola was the actual cause remains unclear — the products weren't analyzed for contaminants, and doses weren't recorded. A clinical trial monitoring liver function found no changes at 120 mg daily for 6 months, but caution is warranted.

Cns Depressants
Theoretically, taking gotu kola might increase the sedative effects of CNS depressants.
In vitro research suggests that gotu kola may have sedative effects via binding of GABA receptors.
Hepatotoxic Drugs
Theoretically, taking gotu kola with hepatotoxic drugs might have additive adverse effects.
There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.
