There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.

Here's the thing to keep an eye on with this combo. The acetaminophen part of your Tylenol Cough product can be hard on the liver if you take too much. Gotu kola has been linked to possible liver irritation in a small handful of case reports, so the worry is that the two together could add a little extra stress on your liver. This isn't about gotu kola changing how the drug is broken down or how strong the cough medicine feels. It's simply two things that may each touch the liver.

I'll be honest, the evidence here is weak (just a few anecdotal reports), so for most people taking normal doses this is a low-level concern. Still, watch for signs like yellowing skin, dark urine, belly pain, or unusual tiredness, stick to the acetaminophen dose on the label, and check with your pharmacist if you take it often.
