This product contains one active ingredient: horse chestnut nut extract. Horse chestnut has been studied for chronic venous insufficiency — a condition where veins in your legs don't return blood to your heart as well as they should — and the extract is prepared by processing the seed to remove a toxic component called esculin.
The liquid also contains alcohol, water, and glycerin as inactive ingredients to deliver the extract.

Horse chestnut extract is possibly effective for chronic venous insufficiency based on available evidence. For other uses — hemorrhoids, irritable bowel syndrome, and kidney stones — there isn't enough reliable evidence yet to say whether it works.
How safe is it?
Well-documented data
A properly processed seed extract is generally well tolerated when taken by mouth. The most common side effects are dizziness, gastrointestinal upset, headache, and itching.
One case report describes a serious heart condition (pericardial tamponade) in a man who consumed a large amount of horse chestnut paste over 6 weeks, though the link to the supplement isn't certain. Calf spasms have also been reported in people taking it for vein problems.
Horse chestnut is not safe in pregnancy or while breastfeeding — there isn't enough safety data, so it's best avoided during both.

Horse chestnut contains the constituent esculin which has been shown to have antithrombotic effects. Therefore, horse chestnut might have antiplatelet effects. This has not been shown in humans.
