Idebenone is generally well tolerated in studies. Uncommon side effects include nausea, vomiting, abdominal pain, elevated alkaline phosphatase (a liver enzyme), and loose stools.

One person reported a faster heart rate (tachycardia) in a trial, but it didn't require treatment. High doses up to 2,250 mg daily have been well tolerated without a significant increase in side effects.

There is a case report of an allergic skin reaction (contact dermatitis) from topical use of a cream containing idebenone. There is not enough safety information to use idebenone during pregnancy or while breastfeeding — the data advises against it.
