Indole-3-carbinol from food is generally well tolerated, but supplement doses at higher concentrations are less studied and should be used with caution. The most common side effects reported are diarrhea, nausea, rash, and tremor.

Some people have experienced unsteadiness and imbalance, especially at higher doses or when doses were not followed correctly. One adult taking 400 mg twice daily developed imbalance and tremor after 10 days—it resolved when the dose was halved.

Because of possible hormone effects and limited safety data, concentrated supplements are not advised during pregnancy, and they are not recommended while breastfeeding.
