What’s inside
Full disclosure
GNC Total Lean L-Carnitine 1000 contains one active ingredient: L-carnitine, an amino acid your body naturally makes and uses to convert fat into energy. The capsule also contains microcrystalline cellulose, stearic acid, magnesium stearate, silicon dioxide, and a vegetarian coating — these are inactive ingredients that help hold the capsule together.
Does it work?
Not established
L-carnitine is effective for treating L-carnitine deficiency — a real but uncommon condition. The evidence also suggests it's possibly effective for high cholesterol (hyperlipidemia), congestive heart failure, and angina.
That means the research shows a benefit, but the evidence isn't as strong or complete as we'd like. If you're considering this for any of those conditions, talk with your doctor about whether it makes sense for your situation.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at typical doses. The most common side effects are stomach upset — including abdominal cramps, pain, nausea, diarrhea, and heartburn — and a fish-like body odor, which some people report.
High doses are more likely to cause these effects. Seizures have been reported rarely.
Pregnancy data isn't on file, so if you're pregnant or planning to be, talk with your doctor or pharmacist before taking this. L-carnitine is possibly safe while breastfeeding, but the evidence is limited; check with your doctor or pharmacist to be sure it's right for you.
Meds to double-check
Moderate interaction found
Before you start, check with your doctor or pharmacist if you take blood thinners like warfarin or acenocoumarol (Moderate interaction — L-carnitine may increase their effects). Also confirm if you're on thyroid hormone replacement (Moderate interaction — L-carnitine may reduce how well it works).
The bottom line
Scorecard at a glanceFully disclosed formula with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.
L-carnitine works best if you have a confirmed L-carnitine deficiency or want to explore it for heart health under medical guidance. If you take blood thinners or thyroid medication, you'll need to check with your doctor or pharmacist first — this product can affect how both work.
Stomach upset and fishy odor are the main things to watch for at typical doses.

L-Carnitine 1000 by GNC Total Lean Drug Interactions
HelloPharmacist Interaction Report
GNC Total Lean L-Carnitine 1000 does interact with medications, primarily through its single active ingredient, L-carnitine, which affects blood thinners and thyroid hormones.
Read the full breakdown — every affected drug type, severity by severity
The most serious concern is with blood thinners (anticoagulants). L-carnitine may increase the effects of warfarin (Coumadin) and acenocoumarol (Sintrom), meaning your blood may thin more than intended — a Moderate severity interaction.
There are documented case reports of elevated INR (a measure of how thin your blood is) when L-carnitine was used alongside acenocoumarol.
L-carnitine also interacts with thyroid hormone replacement medication at Moderate severity. It appears to work against how your body uses thyroid hormone, potentially decreasing its effectiveness.
If you take thyroid medication, this is worth discussing with your doctor or pharmacist before starting this product.
Altogether, these interactions span 19 individual medications. Check your exact prescriptions using the tool on this page before you start.

For each ingredient in L-Carnitine 1000 with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
