What’s inside
Full disclosure
This capsule contains two active ingredients: L-carnitine (275 mg), an amino acid that helps your cells produce energy, and a proprietary herb blend (275 mg). The blend's specific herbs aren't listed on the label, so we can't tell you what's in it.
The capsule itself is made from gelatin.
Does it work?
Strong evidence
L-carnitine is effective for treating L-carnitine deficiency. It's possibly effective for high cholesterol, congestive heart failure (when the heart can't pump blood efficiently), and angina (chest pain from reduced blood flow to the heart).
Keep in mind that the evidence for those three conditions is provisional — the research shows promise, but it's not conclusive. We have no effectiveness data on file for the proprietary blend.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at typical doses. The most common side effects are stomach-related: abdominal cramps, pain, diarrhea, heartburn, nausea, reduced appetite, and vomiting.
Some people also report a fishy body odor. Seizures are rare but serious.
The safety data for this product while breastfeeding is limited, so caution is advised — talk with your doctor or pharmacist about whether it's right for you if you're nursing. Pregnancy safety data isn't on file, so there isn't enough information to know either way — check with your doctor or pharmacist for personalized guidance.
Meds to double-check
Moderate interaction found
Before taking this product, check with your doctor or pharmacist if you take blood thinners like warfarin or acenocoumarol — L-carnitine may boost their blood-thinning effect. Also flag it if you're on thyroid hormone replacement, as L-carnitine may reduce how well it works.
The bottom line
Scorecard at a glanceFully disclosed formula with strong clinical evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
This product may help if you have low L-carnitine levels or are exploring it for heart conditions or cholesterol — but it's not right for you if you take blood thinners or thyroid replacement therapy without checking first. The proprietary blend is a wildcard since its ingredients aren't disclosed.
Talk it over with your doctor or pharmacist before starting, especially if you're on any prescription medications.

For each ingredient in L-Carnitine 275 mg Plus Herb Complex of 275 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
