What’s inside
Full disclosure
This product contains one active ingredient: L-carnitine 500 mg per capsule. L-carnitine is an amino acid that plays a role in energy production by helping your body burn fat for fuel.
The capsule also contains inactive ingredients — cellulose, cellulose powder, magnesium stearate, and silica — which are fillers and binders.
Does it work?
Strong evidence
L-carnitine is effective for treating L-carnitine deficiency. It is possibly effective for high cholesterol (hyperlipidemia), congestive heart failure, and angina, though the evidence for these uses is not conclusive.
If you're considering L-carnitine for a specific condition, talk it over with your doctor about whether the evidence supports its use for your situation.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at typical doses. The most common side effects are stomach-related — abdominal cramps, pain, diarrhea, heartburn, nausea, and reduced appetite — as well as a fishy body odor that some people report.
High doses are more likely to cause stomach upset. Seizures are rare but have been reported.
The safety data while breastfeeding is limited, so caution is advised if you're nursing — talk with your doctor or pharmacist about whether it's right for you.
Meds to double-check
Moderate interaction found
Before taking L-Carnitine 500 mg, check with your doctor or pharmacist if you take a blood thinner (anticoagulant) — warfarin (Coumadin) or acenocoumarol (Sintrom) — as L-carnitine may increase bleeding risk. Also check if you take thyroid hormone replacement therapy, since L-carnitine may reduce its effectiveness.
The bottom line
Scorecard at a glanceFully disclosed formula with clinical evidence supporting its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
L-Carnitine 500 mg may be worth considering if you have a documented L-carnitine deficiency or are exploring it for heart health under your doctor's guidance. If you take a blood thinner like warfarin or acenocoumarol, or thyroid hormone replacement, check with your doctor or pharmacist before starting — this supplement can interfere with both.
Stomach upset is common at higher doses.

General Statements
L-Carnitine is a non-essential amino acid that helps to maintain overall good health by facilitating the transfer of fatty acid groups into the mitochondrial membrane for cellular energy production.* It naturally occurs in red meat and other animal source foods, but we recommend supplementation to obtain optimal levels of this excellent amino acid.

For each ingredient in L-Carnitine 500 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
