What’s inside
Full disclosure
This supplement contains one active ingredient: L-carnitine, an amino acid your body uses to move fatty acids into cells for energy production. It's often used to support energy and heart health.
The tablet also contains inactive ingredients—microcrystalline cellulose, dicalcium phosphate, vegetable cellulose, silica, stearic acid, vegetable magnesium stearate, vegetable glycerin, and titanium dioxide—which are standard binders and fillers.
Does it work?
Strong evidence
L-carnitine is effective for treating L-carnitine deficiency. There's also evidence it's possibly effective for high cholesterol (hyperlipidemia), congestive heart failure, and angina.
If you're considering this supplement for one of those conditions, talk with your doctor about whether it makes sense for your situation.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at typical doses. The most common side effects are stomach-related: abdominal cramps, pain, diarrhea, nausea, heartburn, and reduced appetite.
Some people also report a fish-like body odor. High doses are more likely to cause stomach upset.
Rare but serious side effects include seizures. There isn't enough reliable safety data for use during pregnancy, so that decision requires personalized medical advice.
L-carnitine is possibly safe while breastfeeding, but the data is limited—check with your doctor or pharmacist before using it.
Meds to double-check
Moderate interaction found
Before taking this supplement, check with your doctor or pharmacist if you're on blood thinners—especially acenocoumarol or warfarin (Coumadin)—or thyroid hormone replacement. L-carnitine may increase how much your blood thinner works or make your thyroid medication less effective.
The bottom line
Scorecard at a glanceFully disclosed formula with clinical evidence supporting its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
L-carnitine may help with energy and certain heart conditions, and it's usually well tolerated. If you take blood thinners like warfarin or acenocoumarol, or thyroid hormone replacement, this supplement isn't for you without first checking with your doctor or pharmacist.

L-Carnitine 500 mg by Solgar Drug Interactions
HelloPharmacist Interaction Report
Solgar's L-Carnitine 500 mg interacts with blood thinners and thyroid hormone replacement through its single active ingredient.
The most serious concern is with acenocoumarol (Sintrom), a blood thinner: L-carnitine may enhance its effects and increase bleeding risk, with at least two case reports of elevated INR (a measure of blood thinning). Warfarin (Coumadin), another blood thinner, may also see increased anticoagulant effects, though the evidence is more theoretical.
Read the full breakdown — every affected drug type, severity by severity
L-carnitine may also decrease how well thyroid hormone replacement works by blocking the hormone from entering cells, potentially reducing its effectiveness.
Altogether, these interactions span 19 individual medications. Before starting this supplement, talk with your doctor or pharmacist about any blood thinners or thyroid medications you're taking.

For each ingredient in L-Carnitine 500 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
