What’s inside
Full disclosure
This product contains one active ingredient: L-carnitine fumarate, a compound your body uses to help move fatty acids into cells for energy production. L-carnitine is naturally present in your body and in foods like meat and dairy, and some people's bodies don't make or absorb enough of it on their own.
The capsule itself is made from gelatin, cellulose, magnesium stearate, and silica — standard inactive ingredients that hold the supplement together.
Does it work?
Strong evidence
L-carnitine is effective for treating L-carnitine deficiency — a condition where your body doesn't have enough of this compound. For other uses, the evidence is more mixed: the data shows it's possibly effective for high cholesterol (hyperlipidemia), congestive heart failure (CHF), and chest pain (angina), but "possibly effective" means the research is suggestive, not conclusive.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at normal doses. The most common side effects are digestive — stomach upset, diarrhea, nausea, and heartburn — and a fishy body odor, which some people report.
High doses are more likely to cause stomach problems. Rarely, seizures have been reported, though this is uncommon.
There isn't enough safety data on L-carnitine in pregnancy to know whether it's safe, so talk with your doctor or pharmacist if you're pregnant or planning to become pregnant. For breastfeeding, the evidence is possibly safe but not well studied, so caution is advised — discuss it with your healthcare provider.
Meds to double-check
Moderate interaction found
Before you take this, double-check your medications with your doctor or pharmacist if you're on blood thinners like warfarin (Coumadin) or acenocoumarol (Sintrom) — L-carnitine may strengthen their blood-thinning effect. Also flag it if you're taking thyroid hormone replacement, since L-carnitine may reduce how well it works.
The bottom line
Scorecard at a glanceFully disclosed formula with strong clinical evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
If you have an L-carnitine deficiency (confirmed by testing), this supplement is a solid choice. It's also worth considering if you have heart disease or high cholesterol and want to explore a possibly helpful add-on — but the evidence is preliminary.
If you're on warfarin, acenocoumarol, or thyroid hormone replacement, check with your doctor or pharmacist before starting. Most people tolerate it well, though digestive upset and a fishy odor are common at higher doses.

Formulation
L-Carnitine, an amino acid derivative, is found in nearly all cells of the body. L-carnitine transports long-chain fatty acids across the inner mitochondrial membranes in the mitochondria, where they are processed by beta-oxidation to produce biological energy in the form of adenosine triphosphate or ATP.

For each ingredient in L-Carnitine 500 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Carnitine Fumarate
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
