L-carnitine is effective for treating L-carnitine deficiency — a real condition where your body lacks enough of this amino acid. For high cholesterol (hyperlipidemia), heart failure (congestive heart failure or CHF), and angina (chest pain from reduced blood flow to the heart), the evidence is possibly effective — meaning there's some promising research, but it's not yet strong enough to call it proven.

How safe is it?

L-carnitine is generally well tolerated at typical doses. The most common side effects are digestive — stomach cramps, diarrhea, nausea, heartburn, and reduced appetite.

GNC Women's L-Carnitine interacts with blood thinners and thyroid hormones.
The most serious concern is with acenocoumarol (Sintrom), a blood thinner similar to warfarin — L-carnitine may boost its anticoagulant effect, and there are at least two case reports of increased INR (a measure of blood thinning) when the two were taken together. This is a Moderate severity interaction.

L-carnitine may also increase how much warfarin (Coumadin) thins your blood, though the evidence is mainly theoretical at this point. If you take either of these blood thinners, your doctor will need to monitor you closely and may adjust your dose.
Thyroid hormone replacement (levothyroxine and similar drugs) is another concern at Moderate severity. L-carnitine appears to work against thyroid hormone by blocking its entry into cells, which could reduce how well your thyroid medication works.

L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
