What is this page for?
First and foremost: checking L-Carnitine against your medications. The heart of this page is the interaction checker and the full interaction report — how this product’s ingredients may interact with prescription and over-the-counter medicines you may be taking.
Around that, we add a pharmacist’s high-level view of the product as a whole — what’s inside, the evidence for its stated use, how transparent the label is, and what safety data exists — so you can see the full picture in one place. It’s educational information from our licensed clinical databases and the clinical staff at HelloPharmacist — not medical advice — and we don’t sell or endorse products. Our editorial policy

What’s inside
Full disclosure
RSP L-Carnitine is a single-ingredient supplement in capsule form. L-carnitine is an amino acid-like compound your body makes naturally and that plays a role in how your cells produce energy.
The capsules also contain inactive ingredients—gelatin, plant cellulose, silicon dioxide, and magnesium stearate—which are standard binders and flow agents.
Does it work?
Strong evidence
L-carnitine is considered effective for treating L-carnitine deficiency, a rare condition where your body doesn't make or absorb enough of this compound. For other uses—high cholesterol (hyperlipidemia), heart failure (congestive heart failure), and chest pain (angina)—the evidence is rated possibly effective, meaning some research suggests benefit, but more study is needed to be sure.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at typical doses. The most common side effects are stomach-related: abdominal cramps, pain, diarrhea, heartburn, nausea, and reduced appetite.
Some people also report a fishy body odor. High doses are more likely to cause stomach upset.
Rarely, seizures have been reported. The safety data while breastfeeding is limited, so caution is advised if you're nursing—talk with your doctor or pharmacist about whether it's right for you.
Pregnancy safety information isn't on file for this ingredient, so discuss it with your healthcare provider if you're pregnant or planning to be.
Meds to double-check
Moderate interaction found
Before you start L-carnitine, double-check with your doctor or pharmacist if you take blood thinners such as warfarin (Coumadin) or acenocoumarol (Sintrom)—L-carnitine may strengthen their effect and raise your bleeding risk. Also flag it if you're on thyroid hormone replacement for an underactive thyroid, since L-carnitine might make your medication less effective.
The bottom line
Scorecard at a glanceFully disclosed formula with clinical evidence supporting its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
L-carnitine works best if you have a confirmed deficiency or want to explore it for heart-related conditions under medical guidance. If you take blood thinners or thyroid hormone, check with your doctor or pharmacist first—this supplement may affect how those medications work.
It's generally easy on your stomach at normal doses, but tell your provider if you're pregnant, nursing, or taking any prescription medications.

L-Carnitine by RSP Drug Interactions
HelloPharmacist Interaction Report
L-Carnitine by RSP does interact with certain medications, primarily through its effects on blood thinners and thyroid hormone.
The most serious concern is with blood thinners (anticoagulants): L-carnitine may increase how well warfarin (Coumadin) and acenocoumarol (Sintrom) work, which could raise your bleeding risk. Case reports exist of INR elevation—a measure of blood thinning—when L-carnitine was used alongside acenocoumarol.
Read the full breakdown — every affected drug type, severity by severity
L-carnitine also appears to work against thyroid hormone replacement therapy. It may block thyroid hormone from entering your cells properly, potentially reducing the effectiveness of your medication if you take it for an underactive thyroid (hypothyroidism).
Although these interactions are documented in the medical literature, the evidence is not yet conclusive—they're labeled as theoretical or based on limited case reports. Still, if you take warfarin, acenocoumarol, or thyroid hormone, talk to your doctor or pharmacist before starting L-carnitine.
Altogether, these interactions span 19 individual medications. Use the interaction checker on this page to verify your exact prescriptions.

For each ingredient in L-Carnitine with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
