Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This liquid supplement contains 2 active ingredients. L-carnitine is an amino acid your body uses to move fatty acids into cells where they're burned for energy.
Pantothenic acid (vitamin B5) supports energy production and helps your body make and break down fats and proteins. The product also contains inactive ingredients — water, acids for flavor, sweetener, and preservatives — that make up the liquid base.
Does it work?
Strong evidence
L-carnitine is effective for L-carnitine deficiency and possibly effective for high cholesterol, congestive heart failure, and angina. Pantothenic acid is effective for pantothenic acid deficiency.
For pantothenic acid and other uses like hay fever or hair loss, the evidence isn't established or is insufficient to know either way.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at typical doses. The most common side effects are stomach-related — nausea, diarrhea, heartburn, abdominal cramps, and reduced appetite.
A fishy body odor has also been reported. At high doses, stomach upset is more likely.
Seizures are rare but serious. Pantothenic acid is considered safe at typical supplement doses with few side effects, though oral use has been linked to diarrhea.
Safety data during breastfeeding for L-carnitine is limited, so caution is advised. Neither ingredient has pregnancy or breastfeeding safety data on file beyond what's noted here — talk with your doctor or pharmacist for personalized advice if you're pregnant or nursing.
Meds to double-check
Moderate interaction found
Check your medications if you take blood thinners like warfarin or acenocoumarol — L-carnitine may boost their anticoagulant effect (Moderate severity). Also double-check if you're on thyroid hormone replacement therapy, as L-carnitine may reduce its effectiveness (Moderate severity).
The bottom line
Scorecard at a glanceFully disclosed formula with clinical evidence supporting its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
This product may be worth considering if you have L-carnitine deficiency or are looking at it for heart health or cholesterol support — though the evidence is mixed. Steer clear if you take blood thinners or thyroid hormone replacement without checking first.
Before you start, run your medications through the checker on this page.

For each ingredient in Leanshots L-Carnitine 3000 Green Apple with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
