For oleic acid, there is possibly effective evidence it may help lower cholesterol levels (hypercholesterolemia) and support coronary heart disease. However, flaxseed oil has shown possibly ineffective evidence for obesity, rheumatoid arthritis, bipolar disorder, and high cholesterol (hyperlipidemia) — meaning the research doesn't support using it for these purposes.

Well-documented data
Flaxseed oil is generally well tolerated by most adults. When taken at around 5 grams daily, a small number of patients report changes in bowel habits, dry mouth, or indigestion.
High doses (30 grams or more per day) may cause loose stools or diarrhea. Rare serious reactions like severe allergic reactions (anaphylaxis) have been reported.
Oleic acid in normal food amounts is considered safe, but concentrated supplement doses haven't been well studied. For pregnancy and breastfeeding, the safety data is limited.
Food amounts of oleic acid are fine, but supplement-level doses lack good safety research. Flaxseed oil has insufficient data during breastfeeding — check with your doctor or pharmacist before use if you're pregnant or nursing.

Barlean's Lignan Flax Oil contains two ingredients with documented interactions: flaxseed oil and oleic acid.
The most serious concern is a Moderate interaction with blood thinners (anticoagulant and antiplatelet drugs). Flaxseed oil may decrease platelet aggregation and increase bleeding time, so combining it with these medications could raise your bleeding risk.

Flaxseed oil also has Moderate interactions with blood pressure medications (antihypertensive drugs). While some studies show flaxseed oil can modestly lower blood pressure, others show no effect — but the theoretical risk of additive lowering exists.
Additionally, if you take ezetimibe (Zetia, a cholesterol medication), flaxseed oil reduces how well your body absorbs alpha-linolenic acid from the supplement.
Oleic acid, another ingredient here, carries a Moderate interaction with diabetes medications. Early research suggests a diet high in oleic acid may lower fasting blood sugar, which could theoretically enhance the effect of your diabetes drugs.

Flaxseed Oil
Anticoagulant/Antiplatelet Drugs
Theoretically, using flaxseed oil in combination with anticoagulant or antiplatelet drugs might have additive effects and increase the risk of bleeding.
Small clinical studies show that consuming flaxseed oil might decrease platelet aggregation and increase bleeding time.
Antihypertensive Drugs
Theoretically, combining flaxseed oil with other antihypertensive drugs might have additive effects and increase the risk of hypotension.
Some clinical evidence suggests that long-term consumption of flaxseed oil can modestly lower systolic and diastolic blood pressure, while other clinical research shows no effect.
Ezetimibe (Zetia)
Concomitant use of flaxseed oil and ezetimibe reduces the absorption of alpha-linolenic acid from flaxseed oil.
In one clinical study, concomitant consumption of ezetimibe 10 mg daily with flaxseed oil 2 grams providing 1 gram of alpha-linolenic acid daily blocked the absorption of alpha-linolenic acid, resulting in an overall reduction in alpha-linolenic plasma levels from baseline.

Theoretically, oleic acid might increase the effects of antidiabetes drugs. Preliminary clinical research in patients with type 2 diabetes taking oral hypoglycemic drugs shows that eating a diet rich in oleic acid from olive oil decreases fasting blood glucose levels when compared to eating a diet rich in linoleic acid from sunflower oil. It is unknown if taking oleic acid supplements would have this effect or if this change is clinically significant. Until more is known, use caution. Dose adjustment may be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (DiaBeta, Glynase PresTab, Micronase), insulin, metformin (Glucophage), pioglitazone (Actos), rosiglitazone (Avandia), and others.
