Grape Seed Extract comes from grape seeds and may help with chronic venous insufficiency (heaviness or swelling in the legs). Policosanol is derived from plant waxes and is studied for circulation and cholesterol support, particularly for leg pain on walking (intermittent claudication).
Red Yeast Rice Extract is a fermented rice product that contains natural lovastatin, a statin-type compound, and is used for cholesterol management.
Does it work?
Strong evidence
Grape Seed Extract is possibly effective for chronic venous insufficiency but possibly ineffective for chemotherapy nausea, hay fever, and weight loss. Policosanol is likely effective for intermittent claudication — pain or cramping in the legs during walking — but the evidence for heart disease prevention is not yet reliable enough to rate.
Red Yeast Rice Extract shows possibly effective evidence for HIV-related cholesterol problems and heart attack prevention. The evidence isn't established in our data for many other uses.
Because this product contains a statin-like ingredient (lovastatin), its effectiveness profile mirrors statin drugs, but the actual strength varies from batch to batch due to inconsistent product quality.
How safe is it?
Well-documented data
Grape Seed Extract is generally well tolerated in studies. Most common side effects are mild — abdominal pain, diarrhea, dry mouth, upset stomach, headache, joint pain, and nausea.
Rare cases of severe allergic reaction (anaphylaxis) to grape skin have been reported. Policosanol is usually well tolerated but may cause headache, mild dizziness, drowsiness, irritability, insomnia, or upset stomach; weight loss has been reported but is uncommon.
Red Yeast Rice Extract is generally well tolerated but carries risks similar to statin drugs: abdominal discomfort, diarrhea, dizziness, gas, headache, heartburn, and muscle pain (myopathy). Rare cases of liver damage (hepatotoxicity) and severe muscle breakdown (rhabdomyolysis) have been reported and are likely tied to its lovastatin content.
Grape Seed Extract concentrated supplements should be avoided in pregnancy due to limited safety data. Policosanol and Red Yeast Rice Extract should be avoided during pregnancy and breastfeeding — there is not enough safety information for either.

Grape Seed Extract carries the most serious concern: it may theoretically reduce the levels of certain medications metabolized by the liver enzyme CYP1A2 (a Moderate interaction), and a small study showed it decreased phenacetin levels. It also may increase levels of drugs processed by CYP2D6 and has conflicting evidence about CYP3A4, all Moderate severity.

Additionally, Grape Seed Extract and Policosanol both theoretically increase bleeding risk with anticoagulants or blood thinners like warfarin (Moderate). Policosanol may cause additive blood pressure-lowering effects with beta-blockers or blood pressure medications like nitroprusside (Moderate), and may increase the risk of low blood sugar (hypoglycemia) when combined with diabetes medications (Moderate).
Red Yeast Rice Extract — which contains the statin drug lovastatin — carries Moderate interactions with other cholesterol medications (statins), liver-damaging drugs, and certain combinations (cyclosporine, gemfibrozil, high-dose niacin) that raise the risk of muscle damage (myopathy) or severe muscle breakdown (rhabdomyolysis).
Although Grape Seed Extract may theoretically reduce cyclosporine absorption, the interaction is Moderate and separating doses by at least 2 hours may help. We could not check Roundhead Lespedeza Extract for interactions — it has no data on file.

Grape Seed Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, grape extracts may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro evidence suggests that grape extracts might decrease platelet aggregation.
Cyclosporine (Neoral, Sandimmune)
Ingesting grape juice with cyclosporine can reduce cyclosporine absorption.
A small pharmacokinetic study in healthy young adults shows that intake of purple grape juice 200 mL along with cyclosporine can decrease the absorption of cyclosporine by up to 30% when compared with water. Separate doses of grape juice and cyclosporine by at least 2 hours to avoid this interaction.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, grape juice might reduce the levels of CYP1A2 substrates.
A small pharmacokinetic study in healthy adults shows that ingestion of 200 mL of grape juice decreases phenacetin plasma levels. This is thought to be due to induction of CYP1A2.
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, grape seed extract may increase the levels of CYP2D6 substrates.
In vitro evidence suggests that grape seed extract might inhibit CYP2D6 enzymes. However, this interaction has not been reported in humans.
Cytochrome P450 2E1 (Cyp2E1) Substrates
Theoretically, grape seed extract might increase the levels of CYP2E1 substrates.
In vitro and animal research suggests that grape seed proanthocyanidin extract inhibits CYP2E1 enzymes. However, this interaction has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
It is unclear if grape seed extract inhibits or induces CYP3A4; research is conflicting.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.
Midazolam (Versed)
Theoretically, long-term intake of grape seed extract might decrease the effects of midazolam.
Animal research shows that subchronic ingestions of grape seed extract can increase the elimination of intravenous midazolam by increasing hepatic CYP3A4 activity. Single doses of grape seed extract do not appear to affect midazolam elimination.
Phenacetin
Grape juice might decrease phenacetin absorption.
A small pharmacokinetic study in healthy adults shows that ingestion of 200 mL of grape juice decreases phenacetin plasma levels. This is thought to be due to induction of cytochrome P450 1A2 (CYP1A2).
Cytochrome P450 2C9 (Cyp2C9) Substrates
It is unclear if grape juice or grape seed extract inhibits CYP2C9; research is conflicting.
In vitro evidence shows that grape seed extract or grape juice might inhibit CYP2C9 enzymes. However, a small pharmacokinetic study in healthy adults shows that drinking 8 ounces of grape juice once does not affect the clearance of flurbiprofen, a probe-drug for CYP2C9 metabolism. The effects of continued grape juice consumption are unclear.
Red Yeast Rice Extract
Cyclosporine (Neoral, Sandimmune)
Theoretically, taking red yeast rice in combination with cyclosporine might increase the risk of myopathy.
Red yeast rice contains varying levels of the statin drug lovastatin. Cyclosporine has been reported to increase plasma levels of lovastatin by 5- to 20-fold, resulting in reports of myopathy and rhabdomyolysis.
Cytochrome P450 3A4 (Cyp3A4) Inhibitors
Theoretically, drugs that inhibit the CYP3A4 enzymes might increase levels of lovastatin from red yeast rice.
Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.
Gemfibrozil (Lopid)
Theoretically, taking red yeast rice in combination with gemfibrozil might increase the risk of rhabdomyolysis.
Red yeast rice contains varying levels of the statin drug lovastatin. Gemfibrozil has been reported to increase the risk of rhabdomyolysis when used in combination with lovastatin.
Hepatotoxic Drugs
Theoretically, concomitant use might increase the risk of liver damage.
Red yeast rice contains varying levels of the drug lovastatin. Lovastatin can cause liver damage in some people. Some clinical research suggests that supplements containing red yeast rice might increase liver enzyme levels in some, but not all, participants. Cases of acute hepatitis have been associated with red yeast rice. Combining it with hepatotoxic drugs might further increase this risk.
Hmg-Coa Reductase Inhibitors ("Statins")
Theoretically, taking red yeast rice with other statins might increase the risk of potential adverse effects.
Red yeast rice contains varying levels of the statin drug lovastatin and might result in supratherapeutic levels when used with other statins. Based on evaluation of data from the US Food and Drug Administration's adverse event reporting system (FAERS), it is recommended that red yeast rice products be avoided in people taking prescription statins.
Niacin
Theoretically, taking red yeast rice in combination with high-dose niacin might increase the risk of rhabdomyolysis.
Red yeast rice contains varying levels of the statin drug lovastatin and has been linked to reports of myopathy. Niacin has been reported to increase the risk of rhabdomyolysis when used in combination with lovastatin.
Policosanol
Anticoagulant/Antiplatelet Drugs
Theoretically, taking policosanol with other antiplatelet or anticoagulant drugs might increase the risk of bruising and bleeding.
Some clinical research shows that taking policosanol 10-50 mg daily for 7-15 days can inhibit platelet aggregation in healthy patients. Although, one clinical trial shows that taking policosanol 10 mg twice daily for 2 weeks prior to warfarin dosing does not affect warfarin pharmacokinetics or warfarin response. Furthermore, a study in patients undergoing percutaneous coronary intervention with a drug-eluting stent found that taking policosanol 40 mg plus clopidogrel and aspirin daily for 30 days modestly reduced the risk for minor bleeding events when compared with taking clopidogrel and aspirin alone. Until more is known, use with caution.
Antidiabetes Drugs
Theoretically, policosanol might have additive effects with antidiabetes drugs and increase the risk of hypoglycemia.
Clinical research shows that policosanol can lower blood glucose levels.
Beta-Blockers
Concomitant use of policosanol with beta-blockers can cause additive blood pressure-lowering effects.
Clinical research shows that policosanol 5 mg daily can have additive blood pressure-lowering effects in patients with hypertension who are taking beta-blockers. Also, animal research shows that policosanol can increase the blood-pressure lowering effects of propranolol.
Nitroprusside (Nitropress)
Theoretically, concomitant use of policosanol with nitroprusside might cause additive blood pressure-lowering effects.
Animal research shows that taking policosanol along with nitroprusside can increase the blood pressure-lowering effects of nitroprusside.
Propranolol (Inderal)
Animal research shows that taking policosanol along with propranolol can increase the blood pressure-lowering effects of propranolol.
Warfarin (Coumadin)
Theoretically, taking policosanol with warfarin might increase the risk of bruising and bleeding.
