Acetaminophen, Chlorpheniramine, Phenylpropanolamine, Opium falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Grape interacts with.
A small pharmacokinetic study in healthy adults shows that ingestion of 200 mL of grape juice decreases phenacetin plasma levels. This is thought to be due to induction of CYP1A2.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product (Hista-Derfule) pairs acetaminophen for pain and fever with chlorpheniramine (an antihistamine), phenylpropanolamine (a decongestant), and a small amount of opium for cough or relief. The piece that touches this interaction is the liver enzyme called CYP1A2, which helps process some of these ingredients. The concern on file is that grape juice may gently speed that enzyme up, which could clear a drug a little faster and slightly weaken its effect.
Honestly, this is mostly theoretical and based on one small study, so I wouldn't lose sleep over it. A handful of grapes or a normal glass of juice is very unlikely to matter. Concentrated grape seed extract supplements are less predictable, so check with your pharmacist before adding one, especially since opium-containing products deserve extra care.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

This combo product mixes a pain reliever (acetaminophen), an antihistamine (chlorpheniramine), a decongestant (phenylpropanolamine), and opium, which is an opioid. The opium portion is the piece that leans on the CYP3A4 enzyme to be processed. Grape seed extract might nudge that same enzyme, but honestly the science is all over the place. Lab dishes hint it could block the enzyme (which would let the opioid build up, meaning more drowsiness, constipation, or slowed breathing), while animal studies suggest the opposite (faster clearance, weaker effect).
The key word here is theoretical. This has never actually been reported in people, and eating grapes won't do this. If you're considering a concentrated grape seed supplement, just run it by your pharmacist or doctor first.

In vitro and animal research suggests that grape seed proanthocyanidin extract inhibits CYP2E1 enzymes. However, this interaction has not been reported in humans.

This combination product is interesting because the part that matters most here is the acetaminophen (Tylenol). One of the enzymes your liver uses to process acetaminophen is called CYP2E1. Lab and animal studies hint that concentrated grape seed extract might slow that enzyme down a bit. In theory, slowing it could change how acetaminophen is handled, though this has never actually been shown in real people.
Honestly, I would not lose sleep over this one. The evidence is only test-tube and animal data, and eating grapes is not a concern at all. If you regularly take a strong grape seed supplement along with this medicine, just mention it to your pharmacist, especially since you are also getting drowsy ingredients (chlorpheniramine and opium) in the mix. Never change anything on your own.
