Acetaminophen, Dextromethorphan, Guaifenesin, Phenylephrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Grape interacts with.
A small pharmacokinetic study in healthy adults shows that ingestion of 200 mL of grape juice decreases phenacetin plasma levels. This is thought to be due to induction of CYP1A2.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This is one of those interactions that sounds fancier than it really is. The thinking is that grape juice may slightly speed up an enzyme called CYP1A2, which can clear certain medicines a touch faster. Of the ingredients in this combination cold product, acetaminophen is the one that touches that enzyme pathway, so in theory grape could nudge its levels down a bit. That might mean slightly less fever or pain relief, not a dangerous spike.
Honestly, the evidence here is thin and theoretical, based on a small study using an older drug. Everyday grapes or a glass of juice are very unlikely to cause a real problem. If you take concentrated grape seed extract regularly, just mention it to your pharmacist or doctor so they can keep an eye on whether your symptom relief feels weaker than expected.

In vitro evidence suggests that grape seed extract might inhibit CYP2D6 enzymes. However, this interaction has not been reported in humans.

Out of the four ingredients in this combo cold product, the one that matters here is dextromethorphan, the cough suppressant. Your body clears it using an enzyme called CYP2D6. In test-tube studies, concentrated grape seed extract appeared to slow that enzyme down a little. If that happened in a person, dextromethorphan could build up and lead to more of its usual effects, like drowsiness, dizziness, or a foggy, lightheaded feeling.
The important thing: this is only lab-based evidence and has not been seen in real people. Eating grapes won't do this. The acetaminophen, guaifenesin, and phenylephrine pieces aren't really affected. Still, if you regularly take a grape seed supplement, just run it by your pharmacist or doctor so they can check the full picture for you.

In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

This is a combination cold-and-cough product. The piece that touches grape here is mainly dextromethorphan, the cough suppressant, which is partly handled by a liver enzyme called CYP3A4. Grape seed extract might nudge that enzyme one way or the other, but honestly the science is conflicting. Lab tests hint it could slow the enzyme, while animal studies suggest it could speed it up.
If it slowed things down, dextromethorphan could linger a bit longer and you might feel more drowsy or dizzy. The important part: this has never actually been reported in people, and it comes only from test-tube and animal data. Eating grapes is no concern at all. If you regularly take a concentrated grape seed supplement, just mention it to your pharmacist.

Here's the piece that matters in this cold-and-cough combo: the acetaminophen (Tylenol) portion is partly handled by a liver enzyme called CYP2E1. In lab dishes and animal studies, concentrated grape seed extract appears to slow that enzyme down. In theory, slowing it could leave a little more acetaminophen hanging around in your body.
Now, the honest truth: this has never actually been reported in people, so I wouldn't lose sleep over it. Eating grapes or drinking grape juice won't do this. The bigger thing with any acetaminophen product is simply not doubling up on Tylenol from different sources, since too much is hard on the liver. If you want to use a daily grape seed supplement with this medicine, just run it by your pharmacist first.
