This formula contains six active ingredients chosen for respiratory support. Marshmallow root and mullein leaf are traditional herbs for the respiratory tract, while lobelia leaf has been used historically for breathing and cough, though evidence for its effectiveness is limited.

Not established
The evidence on whether these ingredients actually work is thin. Marshmallow, mullein, chickweed, and lungwort all carry a rating of insufficient reliable evidence — meaning we don't have solid trial data showing they help with cough, bronchitis, respiratory infections, or the other conditions they're used for.
Lobelia is rated possibly ineffective for smoking cessation. Pleurisy root's effectiveness hasn't been rated in our data.

How safe is it?
Well-documented data
Marshmallow and mullein are generally well tolerated for short-term use, but human safety data are limited for both. Chickweed is considered lower-risk short-term, though data are sparse.
Lobelia is the real caution here — it can be toxic at higher doses, causing nausea, vomiting, diarrhea, dizziness, tremors, and throat irritation even at doses as low as 50 mg. At 600 mg or higher, it may cause serious toxicity with sweating, fast heart rate, convulsions, low blood pressure, and potentially death.
Pleurisy root contains digitalis-like compounds and carries a caution for the same reason — it can cause gastrointestinal upset and, in large amounts, digitalis-like poisoning with irregular heartbeat and cardiovascular collapse. One case report noted contact dermatitis from topical chickweed.

Before taking this product, double-check with your doctor or pharmacist if you take digoxin (Lanoxin) for your heart — this is the most serious interaction and can be dangerous. Also flag lithium (for mood), estrogen-based drugs like birth control or hormone therapy, diuretics or water pills, anticoagulants or antiplatelet drugs (blood thinners or aspirin), or any other oral medications, since spacing may be needed.

Marshmallow may increase lithium levels (a mood-stabilizer medication) due to potential diuretic effects, which could lead to toxicity — your lithium dose might need adjustment. Lobelia carries the same lithium risk for the same reason.
Pleurisy root also interacts with estrogen-based drugs (like birth control or hormone therapy) and potassium-depleting water pills (diuretics such as furosemide, hydrochlorothiazide); these combinations may interfere with hormone therapy or raise the risk of heart toxicity from low potassium.
Marshmallow's mucilage might reduce how well oral medications are absorbed — spacing doses 30–60 minutes apart can help. Both marshmallow and mullein may have mild blood-thinning effects and theoretically could add to anticoagulants or antiplatelet drugs (like warfarin or aspirin), though the effect appears modest.

Marshmallow
Lithium
Theoretically, due to potential diuretic effects, marshmallow might reduce excretion and increase levels of lithium.
Marshmallow is thought to have diuretic properties. To avoid lithium toxicity, the dose of lithium might need to be decreased when used with marshmallow.
Anticoagulant/Antiplatelet Drugs
Theoretically, marshmallow flower might have antiplatelet effects.
Animal research suggests that marshmallow flower extract has antiplatelet effects. However, the root and leaf of marshmallow, not the flower, are the plant parts most commonly found in dietary supplements. Theoretically, use of marshmallow flower with anticoagulant/antiplatelet drugs can have additive effects, and might increase the risk for bleeding in some patients.
Oral Drugs
Theoretically, mucilage in marshmallow might impair absorption of oral drugs.
Marshmallow contains mucilage which can affect oral drug absorption. To avoid changes in absorption, take marshmallow 30-60 minutes after oral medications.
Mullein
Anticoagulant/Antiplatelet Drugs
Lab tests on extracts from different parts of common and dense-flowered mullein plants show that they do have some blood-thinning effects, as they increase the time it takes for blood to clot. This is likely due to at least one of the known constituents of muellein, triterpenoid saponins. However, the effects appear to be mild and results have been inconsistent. Extracts from different plant parts have been show to have varying anticoagulant strengths, with water-based ones being more potent than alcohol-based extracts.
Pleurisy Root
Digoxin (Lanoxin)
Pleurisy root contains digitalis-like cardenolide glycosides. Taking pleurisy root in combination with digoxin could increase the risk of adverse effects and toxicity.
Diuretic Drugs
Theoretically, concomitant use of potassium-depleting diuretics and pleurisy root can increase the risk of cardiac glycoside toxicity due to potassium depletion. Some diuretics that can deplete potassium include chlorothiazide (Diuril), chlorthalidone (Thalitone), furosemide (Lasix), hydrochlorothiazide (HCTZ, Hydrodiuril, Microzide), and others.
Estrogens
Theoretically, excessive amounts of pleurisy root might interfere with hormone drug therapy.
Lobelia
Lithium
Lobelia is thought to have diuretic properties. Theoretically, due to these potential diuretic effects, lobelia might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
