How safe is it?
Well-documented data
Marshmallow and mullein are generally well tolerated for short-term use, but human safety data are limited for both. Lobelia is the ingredient with the most serious safety concern — it can be toxic at higher doses, causing nausea, vomiting, diarrhea, dizziness, tremors, and at 600 mg or above, serious effects like rapid heart rate, low blood pressure, seizures, and even death; use caution and stay within recommended doses.
Pleurisy root also contains potentially toxic compounds similar to digitalis and should be used cautiously. Mullein topically and pleurisy root topically can cause skin irritation in some people.
Chickweed is generally considered lower-risk for short-term use.

The most serious interaction we have on file involves pleurisy root and digoxin (Lanoxin), a heart medication — pleurisy root contains compounds similar to digitalis that can increase digoxin toxicity and cause dangerous heart rhythm problems or cardiovascular collapse.

Marshmallow may increase lithium levels (a mood stabilizer) by reducing how your kidneys clear it, raising the risk of lithium poisoning; your doctor may need to adjust your lithium dose. Lobelia carries the same lithium concern through its own diuretic effects.
Pleurisy root also interacts with potassium-depleting diuretics (water pills like furosemide and hydrochlorothiazide), which can worsen heart toxicity risk. Additionally, pleurisy root may theoretically interfere with estrogen-based hormone therapy.
Marshmallow and mullein both carry minor interactions with blood thinners (anticoagulants and antiplatelet drugs); marshmallow may also impair absorption of oral medications if taken at the same time. Chickweed and lungwort were checked and no interactions are documented in our data, though that is not a guarantee none exist.

Marshmallow
Lithium
Theoretically, due to potential diuretic effects, marshmallow might reduce excretion and increase levels of lithium.
Marshmallow is thought to have diuretic properties. To avoid lithium toxicity, the dose of lithium might need to be decreased when used with marshmallow.
Anticoagulant/Antiplatelet Drugs
Theoretically, marshmallow flower might have antiplatelet effects.
Animal research suggests that marshmallow flower extract has antiplatelet effects. However, the root and leaf of marshmallow, not the flower, are the plant parts most commonly found in dietary supplements. Theoretically, use of marshmallow flower with anticoagulant/antiplatelet drugs can have additive effects, and might increase the risk for bleeding in some patients.
Oral Drugs
Theoretically, mucilage in marshmallow might impair absorption of oral drugs.
Marshmallow contains mucilage which can affect oral drug absorption. To avoid changes in absorption, take marshmallow 30-60 minutes after oral medications.
Mullein
Anticoagulant/Antiplatelet Drugs
Lab tests on extracts from different parts of common and dense-flowered mullein plants show that they do have some blood-thinning effects, as they increase the time it takes for blood to clot. This is likely due to at least one of the known constituents of muellein, triterpenoid saponins. However, the effects appear to be mild and results have been inconsistent. Extracts from different plant parts have been show to have varying anticoagulant strengths, with water-based ones being more potent than alcohol-based extracts.
Pleurisy Root
Digoxin (Lanoxin)
Pleurisy root contains digitalis-like cardenolide glycosides. Taking pleurisy root in combination with digoxin could increase the risk of adverse effects and toxicity.
Diuretic Drugs
Theoretically, concomitant use of potassium-depleting diuretics and pleurisy root can increase the risk of cardiac glycoside toxicity due to potassium depletion. Some diuretics that can deplete potassium include chlorothiazide (Diuril), chlorthalidone (Thalitone), furosemide (Lasix), hydrochlorothiazide (HCTZ, Hydrodiuril, Microzide), and others.
Estrogens
Theoretically, excessive amounts of pleurisy root might interfere with hormone drug therapy.
Lobelia
Lithium
Lobelia is thought to have diuretic properties. Theoretically, due to these potential diuretic effects, lobelia might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
